Introduction There is a new term in the nursing profession that i.e. compassion fatigue. The term has been used to describe emotional changes experienced by the nurses during the discharge of their duties. During the provision of care to the patients, nurses observe their patient die, suffering of pain, and their state of helplessness which culminates to serious sense of trauma.

It is the responsibility of every stakeholder in the nursing profession to ensure that quality services are provided to patients. The nurse tends to patients and in the process reassures them of their safety. In order to ensure that there is noteworthy service delivery in the health care setting, it is necessary that every institution should come up with an action plan.

Gibbs's (1988) model will be applied in this critical reflection. This model is meant to establish a specific assessment and analysis of my actions and feelings about the situation. His model includes six phases which are meant to study my actions at each stage of the practice and with each intervention. It is also meant to teach me what I did wrong.

Long terms conditions are significant health issues that often severely compromise the health and the normal functions of individuals.  For some patients, they may often suffer from multiple illnesses and these multiple pathologies usually exacerbate patient symptoms.  This paper shall be a case study of a patient suffering from two long-term conditions.
