The prevalence of various chronic and life threatening diseases has significantly contributed towards raising concern amid people in a considerable manner (Cappuccio, Miller & Lockley, 2010). This paper would focus on understanding the issue of sleeping health amid the practices of APN’s. Identification of the Topic and Target PopulationThe topic that was chosen for this study is Sleep Health, which is one of the prime determinants of ensuring health and wellbeing of individuals.

For the purpose of study, the chosen population is in between the age group of 18-45 years. Description of the Health Promotion DiseaseSleep health is a particular issue that directly relates to the promotion of good health amid individuals. Proper sleep especially for individuals in between the age group of 18-45 years can act as a measure that can ensure maximum stability amid heath of individuals (Cappuccio, Miller & Lockley, 2010). Appropriate sleep for individuals within the age group of 18-45 years can enable them to deal with or control disease such as diabetes and obesity.

It can also result in improving memory of people. Correspondingly, proper treatment should be acquired for dealing with sleeping disorders such as insomnia and restlessness recorded while sleeping. Adequate treatment would ensure proper amount of sleep amid people between the age group of 18-45, further ensuing a better and comprehensive health of the same. Measures must be taken to promote sleep health towards attaining comprehensive benefits for the overall society at large. Relevance of Sleep Health with Advanced Practice Nurse (APN)The role of APN is deemed to be quite crucial for dealing with the issues of sleep health in extensive manner.

Some of the common form of sleeps health issues or disease includes sleep apnoea (OSA), hypopnoea and insomnia among others. The roles of APN in dealing with sleep health issues are characterised by their approach of sleep promotions at large. It has been learnt that APN’s can also be able to provide specialised ad individualised care for the patient with regard to the target respondents, so that various issue can be managed efficiently (Redeker & Phillips, 2011; Cappuccio, Miller & Lockley, 2010).

Theoretical FrameworkThe service-learning model can be considered best suited for the target respondents with regard to health promotions. The model enables nurses to makes effective decisions based on morality and ethicality. The model, which includes steps such as explorations and clarification of information to ensure better decision making will be considered effective in making proper intervention for dealing with the problems of the patients (Cappuccio, Miller & Lockley, 2010).Intervention PlanEpidemiological assessments that can be adopted by nurses with regard to the issue include analysing potential health problems of the targeted population, which is 18-45 years in this case.

Furthermore, with regard to environmental analysis, the surrounding environment of the patient will be judged so that better plan can be developed.
