The Community Social Work Group is a part of the Community and Well-Being Directorate located in the South East of England. The organization mainly works with adults that are people above the age of eighteen with physical disability and with the old people. The team conducts Assessments of Need according to the terms of NHS and Community Care Act 1990 which involves administering of care packages and then reviewing and monitoring if those packages are delivered properly. The emphasis of the organization is to upgrade their packages in the long run which will benefit the service users to a greater extent.

The organizational structure is as follows. It comprises of one Team Manager two Assistant Team Managers, two Senior Practitioners, ten Social Workers, four Assistant Care Managers and a Trainee Social Worker. The administrative support is provided throughout the team by four members of the staff and after the restructuring of the team, three reviewing officers have also become members of the team. The entire team works together to fulfil its mission, which is to provide support to those adults who are with special physical needs.

Following this work, being a social worker I was able to establish full confidence in Mr. X that his identity and privacy will be protected and on the grounds of anonymity Mr. X was able to confide in me all his problems and could explain the various situations which he faced. As I started conversing with Mr. X, he told me that he was utterly frustrated and stressed out and needed some time for him to restore his self. I as a social worker had to keep this situations in mind while handling them, I had to be careful and arrange the treatment in such a way that the sentiments of Mr.
