PERFORMANCE IMPROVEMENT PLAN FOR SAN FRANCISCO MEDICAL CENTRE Adm No. Executive Summary Introduction Brief description of organization and services offered San Francisco Medical centre is located 15 minutes from the Downtown Francisco. The medical centre has offered therapy, wellness services and occupational medicine for over ten years. San Francisco offers positive and professional atmosphere in which one can recover quickly. The medical centre has four hospitals, which are located in different parts of the city. All the hospitals serve a specific purpose so as to cater for the needs of the individuals in San Francisco. There are various services offered in San Francisco Medical Centre, and they work towards ensuring that they provide better services to the patients. New Program Initiative Opportunity Values of the organisation include providing a teamwork atmosphere, education, caring care, excellence and quality, and compassionate. The other values include honesty, integrity, honesty, and ensuring that they work with the community. There are three main values that the medical centre focuses which are stewardship, excellence, and dignity(Lewis, Dirksen, Heitkemper, & Bucher, 2014). In stewardship, the medical centre aims at using the resources available to promote healing and wholeness. It also promotes dignity which is the inherent worth and value of a person. Additionally, it comes up with exceeding expectations through innovation and teamwork. Evidenced-based practice is very important in performance improvement. For an organization to ensure that all the requirements are fulfilled, they must come up with an evidenced based practice. The need for healthcare transformation underscores the need for having an effective, efficient and safe management plan. If a company will achieve its organizational goals, there must be a proper evidence-based practice. The evidence-based practice aims at hardwiring the current knowledge in the nursing sector to improve patient outcomes and care processes. Evidenced based practice promotes the quality of the healthcare services (Williams & Hopper, 2013). Additionally, the evidence-based practice is critical in creating an excellent culture among the staff. The organizational culture greatly determines the success of an organization. One of the objectives of the healthcare is to improve the patient’s functional abilities, thus improving the quality of life (Francis, 2000). Through performing the functional assessment, nurses determine the current level of the healthcare and improve it. The growing need for better health care services promotes the implementation of the evidence-based practice. In San Franco Medical Care, the societal demands involve the provision of safety, equality, accountability in the provision of health care services. Ideally, the main objective of the proposed performance improvement is to standardize all the health care practices and produce relevant health outcomes. The evidence-based practice increases the demand for quality health care and demand for accountability and safety. Through the evidenced-based practice, the healthcare system makes decisions about a specific population or a group of the patients. Evidence-based practice is also an important tool which leads to research utilization and nursing research (Smith & Parker, 2013). Improvement of the performance of the medical centers focuses on satisfaction, cost outcomes, and processes. The management policies are not designed to generate scientific knowledge, but to improve the operation of the health care. To maintain and improve quality, clinical teams must ensure that they blend analysis and redesign the care within the clinical boundaries. Improving the management ensures that the image of the medical center is maintained regardless of the stiff competition. Benefits and Market Served San Francisco is a diverse city with a population of 850,000 people. People from all other parts work in the city. Hence, the hospitals are located in areas where they can access the services. The medical centre serves people of different backgrounds and status such as low-income population. The medical centre has been licenced to cater for 275 inpatient beds(Tsai & Forbes, 2009). The hospital houses acute and post-acute medical care services. Other services offered include hospice and home services. It also offers outpatient hospital care. Some of the inpatient units include cardio, kidney/ liver and children’s unit. Notably, all the beds in the medical centre are capable of telemetry monitoring. Consequently, the mission of San Franco Medical centre is to dedicate their resources in offering affordable health services to the poor and partnering with the community to improve their quality of life. The goal of the medical centre is to provide quality programs and care and set the community standards. The goal of the medical centre aims at providing cost-effective, convenient, and cost-effective services to the patients(Gress, Higashida, & Lawton, 2006). San Franco mission statement provides inspiration, information and direction and guides the organisation in its operations. The vision of the organisation is to provide affordable, quality, and easily accessible patient services. San Franco Medical Centre strives to provide quality, innovative, education and excellent services. The institution does not only provide quality services to the patients but also to the volunteers, staff, physicians, and staff. It is the best place that the staff and volunteers get good experience in their area of specialization. For an organisation to succeed, it has to have good relationship with both external and internal stakeholders(Black & Hawks, 2009). Thus, San Franco Medical Care ensures that it works for the good of both the internal and external stakeholders. Resources Needed The nursing department needs some changes because it is an integral part of the Medical Centre. Changes to be set in the department should ensure that the medical centre becomes a Magnet certified hospital. The plan goes hand in hand with the medical centre mission which is to provide excellent healthcare services. Changes in the nursing department will ensure that the medical centre becomes the best place to receive care. The management system should be restructured and it has to start from the Chief Nursing Officer(Andrews & Lewis, 2007). Additionally, the patient care delivery system should be restructured, and redesigned to meet the needs of the patients. Hiring should be done in the following leadership roles manager for each unit, directors of the critical care area and the inpatient units, educator for each unit and clinical nurse specialist. Through allocating the positions to different people, new innovative ideas are raised to satisfy the needs of the patients. People who get the new positions ensure that the morale goes up. The new management will also ensure that all the finances of the organisation are managed appropriately thus, ensuring that all the finances of San Franco Medical centre are used appropriately. Communication plays a key role in any institution. In the medical centre, it is an important platform that ensured that the patients are satisfied. Managers are supposed to conduct a daily schedule and ensure that communication is done effectively. Communication ensures that there is shared governance and people are encouraged to come up with new initiatives for nurses. The units should work to achieve organisational goals which include patient satisfaction and provision of quality services. Additionally, the nurses require interdisciplinary committees that conduct the activities of the nurses(Andrews & Lewis, 2007). To execute the plan, there are different resources that can be used such as Medical Surgical Telemetry unit. Additionally, a well-laid improvement plan is critical in the administrative part. The other strength is coming up with unique competence in the core areas of the surgical telemetry unit. The improvement plan offers cost-effective strategy which is beneficial to the rest of the medical centre (Daly, Speedy & Jackson, 2003). Weaknesses may arise in the medical, surgical telemetry unit because it may face vulnerable pressures. The weaknesses need close assessment since weaknesses bring negative consequences to the organisation. Some of the weaknesses that may be experienced in the medical centres include low morale on the part of the employees and poor record keeping executed by the organisation(McCray, 2009). The organisation has to look at the advantageous situation that benefits the organisation. Opportunities such as the Medical Surgical Telemetry unit help in fulfilling the needs of the patients and fill the gaps which have not yet been filled. Opportunities better the management by implementing the horizontal and vertical plan(White, Duncan &Baumle, 2013). Parameters Used to Monitor and Measure Success An organisation cannot work in isolation. There are external stakeholders and competitors who influence the management of the organisation. The management should have a plan to forecast on the management plan to make sure they deal with the threats. Some of the threats that the management plan faces include the emergence of better units and shortages of resources to ensure that the implementation plan is successful.
