Nursing Care Delivery Models of Innovative Nursing Care Model: Nurse Caring Delivery Model I have chosen to adopt the Nurse Caring Delivery Model for our organization because of its innovative, resourceful and integral approach to provide care for patients in a way that is “humanly caring” and yet it applies the recent developments and technologies on hospital care (Joynt, 2008). According to Health Workforce Solutions (2008a), this model was driven by the dissatisfaction of nurses on task-oriented care deliveries and lack of consistent methods on care deliveries.

It meant that the quality of care delivery, which is usually implemented in hospitals, is based on recognized clinical tasks depending on the needs of the patients, which suffice the medical requirements for recovery. However, a care system that delivers the same clinical tasks but characterized by “humanly caring” attitudes and initiatives launched by the nurses or caregivers would make a big difference in the patients’ wellbeing. I believe this is an effective philosophy that can be adopted by hospitals to demonstrate its mandate and integral value that is to help patients recover from sicknesses or diseases and develop good health through methods that are faster and of better quality.

In fact, some key elements of this model include nurse team-based care and active patient involvement (Health Workforce Solutions, 2008b). To implement the model, a group of teams with four members each was formed to care for all patients. Each has a team leader who is an expert on delegation, conflict resolution, problem-solving and group-based decision making facilitation. Each group aims to provide care with quality of high levels of interdependence, and communication, collaboration and partnering.

While in the active patient involvement, nurses assures that uninterrupted “caring moment” was delivered to each patient for a period of time and nurses engage patients applying “caring behaviours”. This is an ideal caring model to follow that can benefit both patients and the nursing practitioners, and also the hospital organization itself. As nursing professionalism is promoted to the level of keeping in touch with the needs of the patients, it would allow their faster recovery and good wellbeing.

Evidences of such have been continually reported especially in areas of chronic illnesses, where patients experience health improvements as results of caring methods delivered with commitment, sincerity and concern by the nursing care practitioners. Development Team for Innovative Nursing Care Model The following personnels would be required to implement the Nurse Caring Delivery Model (Health Workforce Solutions, 2008c): 1) A clinical team leader who exercises strong leadership, effective communication, delegation and coordination towards his/her member nurses to bring the whole group to focus on providing prioritized and individualized needs of the patients.

The team leader initiates and educates the members of the team regarding the needs of the patients and the skilled delivery of “caring” interventions. 2) The RN clinical team members who actively help the clinical team leader in management and implementation of the Nurse Caring Delivery Model, and perform his own area of primary care assignment. 3) A patient care associate who is also a part of the clinical team member with a role of helping the RN team members on ADL’s, comfort and hygiene measures.

The patient care associate provides for the needs related to comfort and hygiene of the patients to fasten their medical recovery and enable them to respond to the caring strategies related to wellbeing. These components of the model which is about the persons responsible for the execution of the main objectives of the Nurse Caring Delivery Model are critical to the success of the model itself. The level on how they will perform each caring tasks according to the model is also the level of measurement of how successful the model is going to take effect to the patients.

The nurses and associate members can be engaged into implementing the model by strategies of appropriate trainings and communication. The model is the fulfilment of a good work done by each nurses and associates. It is an innovative method to provide care to the patients. When these concepts are properly communicated to the team and train them to perform their tasks in accordance to the methods of the model, I believe the team will have an appreciation of their higher calling as care providers and this will motivate them to do their part so that good results can be expected, with benefits not only to the patients but them as well, by functioning effectively in their roles.

Incorporation of Selected Model into My Work Setting The Nurse Caring Delivery Model as prescribed in Health Workforce Solutions (2008d) would be adapted to our current work setting by first selecting a strong nurse candidate who would lead the team towards the initiatives and, also measuring the willingness of the staff to implement the methods to create change in the hospital setting, according to the goals of the model. Once this is settled, a hospital area for implementation is also needed to be selected, i.e. an area of 12-bed subunit of a 36-bed medical/surgical unit.

This would serve as the pilot area for the implementation of the Nurse Caring Delivery Model. Important aspects of the model must then be strictly observed. These would encompass an overall excellence of nursing care, concern must be genuinely demonstrated by staff, there must be anticipation of needs by the team, individualization of patient or family education, the provision of care must be conducted with excellent skills, respect must be shown to patients and their families, team members, and other healthcare providers (Health Workforce Solutions, 2008e).

Since communication and partnership are important base of the model, practices with decreased sense of isolation, frequent team communication, commitment to team work and creating a core of nursing practice based on caring, will also be taken into action (Health Workforce Solutions, 2008e). When these areas are done well and results of patients’ satisfaction are achieved, the application of the model can now be extended to another unit of the hospital. When familiarity is acquired in each specific hospital units in conducting the strategies of the model, furthering its implementation until all hospital units are covered will be targeted.

Evaluation of Model: Outcome Measurement Success of the model can be measured through parameters of number of patients, their recovery rate, their satisfaction, their wellbeing status and also, the performance of the clinical team leader and evaluation of the team members. An increasing pattern in positive outcomes must be observed in both the care delivery practitioners and patients. When the model succeeds, satisfied patients can be observed as well as satisfied family members, concerned practitioners are more than willing to provide care for the patients because they will be motivated by the good outcome of the model and therefore, effective provision of the caring needs will be advanced to enable the hospital to provide for a more number of patients.
