Community health assessment is significantly imperative in assisting health practitioners, policy makers, and government to identify and analyze community’s health needs. Additionally, community health assessment is an incredibly vital tool in the identification of resources necessary in addressing emerging health issues as well as in the detection of health needs that demands for immediate action. The health care of mothers, children, and infants section of the population is exceptionally essential in shaping up the future generation as it helps in the prediction of health needs and challenges in future families, communities, and health care system. To understand the maternal, infant, and child health needs in Montclair, New Jersey, it is imperative for Montclair community nurses to evaluate and analyze the community’s health behaviors, the quality of life in Montclair Families, children, and women and the nature and capabilities of the Montclair health systems (Baron, 2005). In most cases, a pregnancy plays an extremely indispensable role in the identification of health needs among pregnant women. Knowledge on pregnancy tests helps in the identification of the most effective means of preventing future health problem among pregnant women and their children. Health risks among Montclair community’s pregnant women include: depression, unhealthy weight, inadequate nutrition, genetic conditions, diabetes, sexually transmitted diseases, and side effects of excessive use of alcohol and tobacco. The risks that are related to pregnancy related complications can be reduced by availing quality pre-conception and inter-conception care. Additionally, mortality and morbidity rate can also be reduced through early identification and treatment of unpleasing health conditions among pregnant women (Center for Disease Control and Prevention, 2003). Some of the universally known factors that interfere with child birth among the Montclair women include: preconceptions, poverty, limited access to inter-conceptions and pre-conceptions as well as age. Child health is as well influenced by socio-demographic factors that include: mental and physical health among parents as well as limited family’s income. Social maternal health determinants among Montclair community entail: pregnancy health status and behaviors brought about by social and environmental factors. Physical factors that affect healthy pregnancy in Montclair community comprises of lack of prenatal care and environmental factors that directly interferes with the mother’s health as well as the ability to employ quality health behaviors. However, lack of adequate measures to address the morbidity and mortality rate brought about by pregnancy complications may significantly contribute in the reduction of the community population. Additionally, cases of children with mental and physical complications may rise significantly. Owing to the good-will of the community’s political elites, an effort to reduce deaths caused by pregnancy complications may be extremely valuable and profitable. The community values and small population may facilitate any efforts to solve the sky-rocketing mortality and morbidity rates. Despite the favorable condition to minimize maternal, infant, and children health complication, lack of commitment among community members may significantly interfere with any plan to counter morbidity and mortality rate in Montclair community. Goals and Objectives The main goal of the plan is to reduce by over 50% the death rate caused by maternal, infant, and children health complications. To achieve this goal, the plan has set an objective of educating the community members on the impact of pre-conception and inter-conception care. Secondly, the plan aims to train community nurses on the contemporary and most effective means of treating pregnancy related problems. Thirdly, the program intends to inform the community members on the need for consistent family planning. To achieve the set objectives and goals, community nurses will conduct public campaigns to educate community members on the significance of family planning in an effort to reduce pregnancy health complications. Secondly, the plan intends to hold several seminars to sensitive the community on the important of pre-conception and inter-conception care. Conferences will also be organized to train community nurses on the emerging and most effective prevention measures. Intervention Primary Intervention The plan intends to educate the community members on the need and significance of taking balanced diet to prevent health complications during pregnancy. The community nurses will be educated on the need of taking instant action to prevent deaths during births. To prevent unplanned pregnancies that might lead to pregnancy complication, the community members will as well be informed on the value of family planning. Secondary Prevention The plan also intends to educate the community members and the nurses on the significance of taking abrupt action to counteract symptoms of health complications during pregnancies. Nurses will be trained on the most effective methods to deal with pregnant mothers who encounter health complication. Finally, community nurses will be informed of the importance of recommending regular screening and testing of patients who exhibit signs of health complications Tertiary Intervention The program also intends to provide means of managing health conditions brought about by pregnancies symptoms. On the other hand, the program plans to encourage mothers to take their children for immunization and screaming. Action phase The plan intends to offer intensive training to community nurses through conducting seminars and conferences. Secondly, to educate the community members on the significance of family planning and pre-conception and inter-conception care, the program will use public rallies, media, and posters. Additionally, the community hospitals and nurses will as well be used to provide relevant tertiary intervention programs. In this stage, the program will use the existing hospitals and community centers to improve the health of the community. The plan is as well estimated to use approximately $1200 for period of one year. The project will also rely on the existing equipment in community’s hospitals. Evaluation This phase is intended to examine whether the plan will achieve its core objectives. The plan intends to use hospital health care database to examine the significant of the plan in countering the increasing mortality and morbidity rate. Secondly, to assess the competence of community nursing, the nurses will be subject to oral and written test. Community members will also be served with questionnaire after full implementation of the plan, to assess their awareness on the importance of pre-conception and inter-conception care as well as the significance of family planning. Appendix Community health assessment offers an intensive understanding on specific community health needs as well as in the identification of the most useful ways of addressing emerging health issues. However, in spite of its encouraging impact on the community health, community assessment process is exceptionally pricey and time consuming. After full implementation of the plan, there was significant transformation on community member’s perception on the significant of family planning and pre-conception and inter-conception care.
