The diagnosis as indicated in the charts shall cover the following tests which ultimately indicate a diagnosis of atherosclerosis: physical exam, blood tests, EKG, chest x-ray, echocardiography, CT-scan, and stress testing. Physicians involved in the diagnosis shall include a cardiologist, a vascular specialist, and a neurologist.
