In the diagnosis, evaluation, treatment, and management of such conditions, it is normally imperative were here required to ensure that the patients are attached to the careers who have the capability of providing them with not only safe but also effective care for the duration of the service. As such every aspect of the management should always be designed in a way that ensures the patient always gets services from practitioners who are capable of giving effective services. This at times requires the formation of a multi-disciplinary team to help out in giving the best possible care.

Procedure

 

  1. Provision of information: Initial identification of needs or risk for assessment and priority. And referred to required agencies.
  2. Referral and initial contact: On receipt of the f refer the rural, the practitioner needs to investigate if the referral is waiting for further assessment (such as change in circumstances, hospital discharge, etc.).
  • Before the initial visit, it is important to investigate e if service user needs help with communication. It is important the to users service user informed about any relevant literature related to service user’s situation.
  1. Initial Assessment: It is essential to gather information in order to ensure immediate safeguarding and support needs are met. 
  2. Also, to identify service user’s strengths and support network this include inviting other relevant parties to the meeting to identify social and individual strengths. First check information held for accuracy.
  3. Establish if there is concern about service user’s mental capacity. If there is then the AMHP duty has to complete a mental health capacity assessment.
  4. In the case where there is no concern about service users’ mental capacity, then I have to complete care management assessment in order to consider how service user will maintain independence. 
    1. Care planning:
    • If the service user is not eligible for services information’s should be offered, advice and guidance of alternative local community support and contact information that may be able to assist them.
