Name Institution Date of Submission Assessment 2 Part A: Case management is a client centered process that is meant to enable a given client with a health problem to meet certain goals in his life through communication. It involves assessing, planning, implementing, coordinating, monitoring and evaluating all the services that are needed or required in order to meet the needs of the particular client. Case management serves many different functions which include: I. It serves to empower a given individual and to ensure that the individual is involved in the planning of the management plan in a dynamic way. II. It also serves to disseminate important information that will be useful in helping and enabling the client to achieve the desired goal. III. Through case management the client’s progress is evaluated enabling the care giver or case manager to make necessary changes if the plan that is being implemented is not bringing about the desired change. IV. The client’s needs are assessed and reassessed through case management. This enables that case manger to check on the progress of the client and if the laid down strategies are beneficial to the client as time goes by. V. Case management also serves to evaluate the ongoing needs of the client VI. It serves the function of monitoring, evaluating and making follow ups that are directed towards checking the progress of the client in regard to eliminating the undesired behavior. My roles as a case manager in an AOD clinic: I. Inspiring and teach the individuals who are being helped by the clinic on how to make the desired changes in their lives and what it will take them to do so. II. I will have to take the role of providing both temporary and long term assistance to clients who come to the clinic seeking to get help. III. My role will also be to help the clients get outside help when need arises through referring them to other centers that will be in a better position to help them IV. As the case manager, I will have to check on the resources that are available in the clinic and decide which ones will benefit the client the most. V. Providing the clients who come to the clinic seeking for help with a practically oriented support that is directed towards enabling them to manage their conditions properly in order to achieve the desired goals. VI. As the case manager in the clinic, I also will have to coordinate all the activities that are designed to help the client meet the desired goals. VII. Service delivery will also be one of my roles in the clinic. I will have to ensure that the services provided meet all the requirements of the clients. VIII. Developing individualized treatment plans for the clients is a role that I have to play as a case manager. IX. Assessment and planning is also a role that I have to play in the clinic. X. Monitoring, evaluating and making necessary follow ups . Tommy’s case: Tommy does face a number of issues. For example, he has a history of alcohol abuse, he has a mental illness, he is aggressive and violent at times. He has a problem of budgeting his finances and spending them well. Instead of him spending his money on things that cold better his life, he spends it in gambling and attending rugby matches. There are different strategies that I could use to engage Tommy in a therapeutic relationship. I have to assure him of confidentiality before we begin the session together. He has to be sure that all his records will be kept confidential. After assuring him of the confidentiality of his records, I will then have to build rapport with him. He will have to talk openly and honestly to me. I will have him understand that I have an objective of helping him get over his problems. Then I will work towards gaining his trust through giving him an assurance that all I’m doing is meant to benefit him and make him a better person in the society. This can be achieved through suspending any moral judgment that will make Tommy feel like a person who cannot be appreciated by the society in general. I also will have to show some empathy, understanding and acceptance. It is necessary for me to do so in order to build the therapeutic relationship Part B: Clients with complex needs have many different social problems which may include lack of a stable accommodation, a history of criminal offences, aggression and may even be violent. They could also be having issues with their self esteem and how they are viewed in society. There fore in order to deal with issues related to these clients, provisions of different services will be a necessity if they are to be helped. They will needed to be provided with temporary accommodation for the period that they are seeking help, they will also have to be given access to rehabilitation centers in case they also have issues with drug and alcohol abuse, they also will need to be given services that will enable them to feel loved and useful to the community at large. Alis’s case: In Ali’s case, he faces a range of alcohol and other drug issues which include: use of cannabis, morphine, alcohol, heroin, and methadone. Initially, I will have to assure Ali of the confidentiality of the whole process. I will also have to warn him against mixing drugs as it will lead to an overdose and eventually into death. I will engage Ali through talking therapy. I will have to discuss with him on the dangers of using drugs the way he does and on sharing his injecting equipment with other people. I will have to talk to him and give him reasons of why he should do away with drugs. I will be able to show empathy to Ali through having a look at the wounds that he has acquired as a result of the injecting kit that he uses, the wounds do not show a sign of drying up which may be a clear indication that he is infected through them. I will then have to console him that all will be alright. In order for me to communicate effectively with Ali, I will have to actively participate in the counseling session that I will be having with him. I will have to listen actively and respond back to any questions that he may be asking appropriately. In the case management process, Ali has a role of following all the procedures, guidelines and sticking to the scheduled visits as it had been laid down in the planning stage. In order for me to set effective goals for Ali, I will have to put into consideration several factors. For one he is an addict of alcohol and other drugs which he injects into his body. He is also somebody who is in employment but does not have a home. Therefore, for me to set effective goals, I will first of all explain to him the benefits of him cleaning out which will eventually lead him to having enough cash that will enable him to acquire a home for himself since he is educated and does not need to live in the streets like most people do. In provision of the case management information to Ali, some factors will have to be put into place. I will have to provide it to him in a way that will not scare him off the clinic. I will start with the most important one of helping him recover from the wounds that do not seem to dry up and move on to getting him a temporary home since he has none. Then information that will enable him deal with the drug problem will be the last one that I will provide him with because it requires a lot of input for him to be able to get over with his drug addictions. Part C: There are many benefits that Ali will benefit from once I refer him to specialized services. For example, if I referred him to a rehabilitation centre, the centre will be able to assist him in getting over with his alcohol and other drug problems. Once he gets over the alcohol and other drug problems, he will be living a life that is free of risks of acquiring diseases from sharing his needles with other people. He will also be able to save enough money that will enable him to have a home. Most of the money that he earns from the job that he does must be going to drugs and that’s why he cannot be able to afford to live in his own. Since he is educated, he will also be in a position of taking his career and educational level to a higher a level. Part D: The effectiveness of the case management process can be checked through evaluation and making up other necessary follow ups. If the clients state of entry remains to be the same throughout the process, then the case management process will be considered as a failure in meeting the desired goals. Some of the evaluation tools in case management include: observation whereby the case manager observes the client to see if there is any change in the way they are leading their lives and comparing it to before they came for help. Reviewing the outcomes of the case management process in order to note all progress that has been made is also part of the evaluation procedure. A case can be closed due to a number of reasons for example, achievement of the set goals, if the client is found to be able to work on the set goals without support, if the client wishes to stop the services of the centre and if the case is transferred to a different service. Mathew’s case: Mathew could be ready for disengagement due to the fact that he has missed his last two appointments and the desire that he has of reducing his alcohol consumption. The relationships that he is building and rebuilding could also be a sign of him being ready for disengagement. I will have to acquire information on his social life in general and any drug or alcohol habits he could be having. his work attendance levels will also provide some information for me. During the disengagement process, some tasks must be completed. I will need to fill in a form on the disengagement process as a record that will have a follow up plan and an alternative disengagement form that will keep a checklist of all the information needed when the client disengages.
