Lecturer Project 4 Checking an Insurance Claim: Write an inspection report H. L Winman and Associates, Buffalo Branch………………………………………………………………475 Reston Avenue, Cleveland, OH 44104FROM: DATE: October 18, 2012TO: Hugh Smithson SUBJECT: Inspection of Damaged Branch Manager ContainersThe truck carrying your containers rolled off the highway 8 miles east of Rochester, New York today at around 12:00 P.M. It belonged to Merryhew Van Lines, Albany.

The waybill number of the containers was C2719I have inspected the containers which are being held at C and J Storage Inc., 63 Crane Street, Rochester to analyze the severity of the damage. I was accompanied by Noella Redovich of Milltown Insurance Company and one of their adjusters. We found the intensity of the damage to be so massive that the containers were beyond repair. Our inspection showed that:• There were 11 containers in total, instead of 12• containers 1,2,3,4,6,8,9,10,11 and 12 are totally beyond repair and have no salvage value remaining• Only container 5 remained undamaged (it must have been cushioned)• Container number 7 was missing (it must have been stolen before the accident)I reported to the police about the missing container at 2:25 P.

MBefore we entrust them with our containers in future, Merryhew Van Lines will have to1. Ensure enough security while transporting the goods2. Check and ensure that all the goods are on board before starting the journey3. Employ fully qualified driversI contacted them and they are willing to do this.I recommend that before the transportation of our containers, one of our people need to be there to ensure that all of them have been shipped.SignatureProject 4.2: Accident at Cormorant DamPart 1Write an incident report to Harry Vincent of the Environmental Protection AgencyH. L. WINMAN AND ASSOCIATES………………………………………………………………475 Reston Avenue, Cleveland, OH 44104TO: Harry Vincent, Regional Inspector FROM:Environmental Protection Agency, Suite 306,444 Waltham Avenue, Cleveland DATE: October 18, 2012 SUBJECT: Report of Air Sampler Box damage I was involved in an accident on September 10, which left me deeply injured and the air sampler box completely damaged.

The accident occurred at 3:15 p.m. while I was standing on a small platform at Cormorant Dam. I had just finished taking the late afternoon air sample measurements and I was in the process of replacing the air sampler in its box. Suddenly, I heard a shout from above followed by two blows, one on my hardhat and another one on my shoulder. The blows were caused by a 3 foot length, 4 inches square construction lumber which had accidentally slipped from above. The lumber knocked off the air sampler box from my hand and the latter rolled to the ground, crashing on the spot.

When I retrieved the box, it was twisted and damaged beyond repair. I was immediately taken to a nearby hospital. The medical examiner confirmed the dislocation on my shoulder. My hand is now in a sling as it cannot support itself. At this point, my left arm cannot grip anything because of the deep injury accrued. The things that were damaged include• The air sampler• The platform I was standing on• The 3 foot length, 4 inches square construction lumber I have shipped the damaged air sampler back to you on Remick Airlines Flight 751.

accompanied with the sampler is the airline’s receipt and the report of the accident. You will pick them at your city’s airport. Prepare it as a memo with a fax cover sheet.H. L. WINMAN AND ASSOCIATESINTER-OFFICE MEMORANDUMTo: M.B. Corrigan From: Fax: Date: October 18, 2012Phone: Pages: 1 Re: Report of theft incidence at Tow Path Inn Cc:Servi Dashi, Fran Pedersen, Sawn Mahler and I were travelling to a power generation construction site in the small town of Freehampton when we encountered a theft incidence.

The town’s location is 508 miles from our main office. The company car we were driving was a Chevy licensed JCP 392. Our travel order was N-704 signed by you 7 days ago. After travelling for 376 miles, we decided to stop for the night at Tow Path Inn, near Whiteshell Lake. It was around 8:05 when we stopped at this small motel, which is located 3.5 miles west of Clearwater Village, on highway A1136. As were having breakfast, Fran Pedersen got up to go and fetch the road map. She returned almost immediately short of breath and reported that the van had been broken into.

We all scrambled from our seats to go and have a look and to our astonishment, we found the driver’s window broken. Shawn Mahler suggested that maybe the thief was after the radio because of the gaping hole at the dash. We checked for the missing items and they are listed below:• The radio• A Nabuchi TX350 “Portacam” Video camera rented from Meadows Electronics at 2120 Grassmere (Serial number 21784B)• Video tapes• $6 from a tray in the boxWe immediately called the Clearwater Police Station and reported the incidence.

The police however requested us to make the report in person for easier investigation. We are requesting for another camera because of its importance. We are supposed to use it for live recording of the construction progress which will be shown at the Power Authority Directors’ Meeting scheduled for 15th of next month. The video camera should be shipped the day after tomorrow by Greyhound bus which passes through Freehampton once a day. We willdrive to Freehampton to meet the bus, and will telephone you tomorrow to check if the video camera will be on that particular bus.
