Concept of Therapy Therapeutic Alliance refers to the relationship between the patient and the mental health therapist. This is one of the most fundamental and collaborative aspect of the relationship between the therapist and the client. It is suggest that for the mental well being it is important for the therapist and client to have a comfortable relationship to achieve the goal. E.S Bordin proposed the concept of Therapeutic alliance in 1979. The strong therapeutic relationship between the healthcare professional and client can be highly beneficial, as it results in effective and better recovery. The concept lays on the idea that the client and the patient are engaged with each other for the beneficial change in the client. Analyzing the case it can be determined that the Therapeutic relationship between the client and the patient it shall be noted that they had a better relationship with each other, that got weaker with the time as reflect ‘Nothing much to report. Work is busy. What else is new? A lot has happened; I guess (Corsini and Wedding)’. It shall be noted in the case that the patient during the session have gradually avoiding the sessions, even during the session the patient is incommunicative with the therapist. Transference Transference refers to the redirection of the patients feeling with the therapist. it is associated with the erotic attraction towards the therapist that can be either in form of mistrust, extreme dependency, rage etc. it can be observed when the patient comes across resistant behavior from the therapist with his patient. it can lead to the persist resistance of the patient towards the session, or complete denial to proceed the therapy. Transference between the patient can often happen between the patient and the therapist unconsciously, this is because of the reason that the patient and client interact with openness. In addition, transference can allow therapist to reveal the unsolved conflicts the child had in the childhood. In the case the patient seems to be dishearten about the therapist behavior when she was telling something serious about her life and she answered the phone call, due to which she says in the session ‘Nothing much to report. Work is busy. What else is new? A lot has happened I guess (Corsini and Wedding)’. it is evident from the statement that the patient has felt left over with the call the therapist received during the session. It made the patient feel that the therapist was least concerned about the important matters she was discussing with her, due to which the patient was reluctant and ignored the sessions. Counter transference Counter transference is the attachment or emotion entanglement of the patient with the therapist. It redirects the feeling of the therapist towards the patient with the emotional entanglement. Counter transference plays a significant role to look into deeper and concern into the matters and issues the patient has been suffering through. It regulates the therapeutic relationship between the client and the patient stimulating their emotions towards the session. Counter transference has been observed between the patient and the client in case, this is evident when the therapist asks ‘But I do remember just few weeks ago you talking about some really important things. I wonder what happened in between? I must have done something or said something, that made it feel different here (Corsini and Wedding)’. This is evident from the statement after the refusal of the session, when the patient was back for the session. Therapist determines a neglected and least concerned answer to the therapists question. This shows that the therapist was emotionally attached with the client, as soon as she sees this straying behavior of the patient she realizes that she must have done something wrong that hurt the client. It is evident when the client replies to the therapist when she was asked about if something went wrong ‘as if that matters (Corsini and Wedding)’. Enactment Individual is always in the process of self-formation, in which they develop rules for the appropriate behavior for the stability in the life, this is called as enactment. In the process of enactment individual transform their behavior in a manner to regain coordination and continuity among the people with each other. This is because of the reason that the people are conscious about the relationship. Enactment allows the patient to overcome the negative and misunderstand one has developed for the particular occurrence. The negativity among an individual consciousness can be overcome with the process of consist talks that have been problematic for the patient. it allows the patient to be vocal about the fear and pain the patient has been gone through in the phase of their life. it acknowledges any difficulties that have been arouse with the acceptance of the patient’s feeling that has went wrong. Enactment can be an effective and beneficial for the treatment outcomes especially when the patient is going negative expectation from the relationship. In the case, the client has been suffering with the issues to overcome the negative aspects of the relationship they have been through. Such as the patient was uncomfortable and faces several issues to engage her in a relationship with the other man, after being divorce for 16 years. Empathy Empathy can be defined as a experience to understand other’s feeling and condition they undergo with their perspectives. Therapists often use the element of empathy in the session to make the patient more comfortable and to ensure that he understands the issue with the perspective of the patient. In addition, it allows the patient to uncover and reveal their problems expressively. Analyzing the case it shall be noted that initially the patient was uncomfortable expressing her feeling freely to the therapist. Gradually the therapist makes the patient more comfortable to express feeling through being friendly and developing proximity with the patient. the therapist develops confidentiality with the client with protecting with the intrusion like ‘saving of the hours’ for the patient. Giving her extra attention and showing her dedication and concern towards the pain and fear the patient has been suffering. The fluctuation between the patient and the client occurred when the therapist seem to pay attention to herself more than the patient. the priorities therapist pays for her life, distracts the empathy between the therapist and the client. It has been evident from the case when the therapist apologizes for the phone call she receives during the session. Value of Termination To determine the value of termination is one of the most crucial part of the session. When the goal of expressive work and personal transformation take place and the patient is able to overcome the fears, talk about them freely and accepting it a part of her life by gaining understanding about the parts of odds. When the therapist finds that the client is gradually overcoming and gaining self understanding about the issues and is more aware with the ways she has been though. The time the patient is strong enough to shift her expectation towards more positive things than the past. She can look in to the future, thinking what she really wants than what has really happened with her, defending herself with the fears is the time when the patient is valued for termination. The maladaptive defense mechanism the patient has developed is the time when the patient’s sessions are terminated (Corsini and Wedding).
