SYSTEM REVIEW EDITING Introduction As a physical therapist, the duty is to provide health care services to maintain, restore, and enhance movement, activity, and health facilitating persons of varied ages to have improved functioning and quality of health, while making sure patient safety and implementing evidence to give efficient and effective healthcare care.1 This report is a summary of the results including findings that would indicate a need for referral to another health care provider. The report is accompanied with a complete history and systems review on a volunteer outpatient subject.

Due to patient confidentiality matter the subject would be named Patient X and addresses and contacts not disclosed. Patient X results provide a broad perspective over which a number of issues can be derived. Patient X history and system review findings shows there is a need for reference to a therapy session in another health care provider. History and System Review Findings The social history of Patient X reveals a married person, has not taken advanced directive, skilled labor, quit smoking, moderate wine taker, fair health condition but no exercise involved.

According to patient X, he has a fair health condition to be assessed during therapy session. The fair state of the patient’s health can be traced to the inherent social behaviors. Patient X is a moderate wine taker; however, astonishing fact is patient X does not involve in exercises either on a voluntary basis or on prompt.2 The family history reveals the father succumbed to cardiac arrest due prolonged heart disease. The mother had passed on due to a cancerous disease. Heart disease and cancer are hereditary and due to family history, there is a need for Patient X referral to another health care provider for further checkups.

3 Patient X has had chest problems on different occasions; this could explain the various breathing problems that the patient has had in the past. Patient X’s current condition shows some inherent complications related to blood sugar levels. In essence, the patient has suffered the complications for a period of six months. Going by the results observed in the outpatient form, there have been some attempts to provide the best medical care to the patient though challenges seem to have hampered the improvement rate in so far as blood sugar is concerned; thus referral is inevitable.

4 The outpatient form denotes that there have been prescriptions given to the patient and for which there have been some medical examination. Some of the drugs that the patient has been taking include the decongestants; to relieve nasal blocking in the upper respiratory tract. The outpatient results provide a broad perspective over which a number of issues can be painted. At the outpatient stage, the idea is to screen the patient and come with some conclusive results capable of ensuring proper diagnosis of the problem in question.

Conclusion The physiotherapy action is aimed at ensuring that the problem is dealt with at the preliminary stages to stop further spread of the disease5. Due to sluggish decision making it is necessary for Patient X to be referred to another healthcare provider for further assessment and treatment.
