If not who provides the social services?Response: Liberty Dialysis has two clinics in Laredo. Each clinic has a full-time Licensed Master Social Worker and one clinic has a part-time LMSW. Thus at any given time, there is a Licensed Master Social Worker at the clinic to offer the social services to the clients.4. Company’s Mission Statement“Liberty Dialysis is dedicated to providing superior care for patients with kidney disease.

We strive to achieve both outstanding quality outcomes for our patients, and a rewarding career experience for our caregivers, all in an atmosphere of compassion, professionalism, empathy and respect. How / and when was this agency established?

Liberty Dialysis started with the idea of offering patients an exceptional care giving in the society, since this field remains unattended for in the American society. This company’s establishment was in 2002 and it is located in the Mercer Island, Washington Liberty Dialysis, together with its affiliates, develops, owns, and operates the dialysis clinics in US. The company offers chronic kidney disease outreach services and home based dialysis programs. The idea behind its establishment is that same dialysis treatment that the outpatient facilities offer to the patients can also be possible just in the safe and effective manner at the comfort of the patient’s home.

More so, studies show that offering home dialysis to the patients improves the sense of well-being and allows room for higher quality life that has fewer risks of hospitalization and complications. Liberty Dialysis is therefore dedicated into providing exceptional dialysis experience for all patients (Bloomberg Business Week, 2013). Some of the services that Liberty Dialysis provides are;(i) Physician-driven teams that focuses on every single patient at a time(ii) Caregivers who are experienced, professional and compassionate(iii) Appointing of the dialysis facilities which meet the state-of –the-art equipment(iv) Offering community-based nephrologists(v) Offering the Chronic Kidney Disease Outreach Programs(vi) Becoming a leading industry that offers patient amenities that include heated massage chairs, then wireless internet access, roaming laptops and some individual television set for each patient.(b) For how long have you been in Laredo TX?

Response: Has been in Laredo, TX for about three years now.6. Describe the population that receives services of the companyResponse: Those in need of the dialysis since their kidneys have lost function. Majority of the patients are waiting for a transplant. Another concept is that most elderly people are the ones that are most affected by kidney problems. This population requires close attention from the family members and special services from a Licensed Master Social Worker. Liberty Dialysis signs deals with qualified local physicians and non-profit organizations that in effect ensures that the needs of the patients are put first to ensure prompt and quality services are available to all. 7. Describe the needs of this populationResponse: Majority of the patients is elderly or at least has a family history of diabetes and high blood pressure.

The youngest patient that I have been able to work with as a social worker was 20 years old. This implies that issues of diabetes and high blood pressure less affect the young generation. 8. How does this agency attempt to meet these needs?Response: The social worker will do an assessment and assist the patient in applying for benefits that they may be eligible to receive benefits and help them settle their bills. It is advisable that people should invest enough on medical pensions that will support them at retirement age.

This will also be effective at avoiding the fact that most of this population depends on the working generation and hence derailing the economic activities. Members who have been paying medical pensions will receive services at subsidized prices hence making the medication process affordable. 9. What are the requirements to receive services at this agency?Response: The patient must be able to pay for services and be referred from their doctor.10. How is eligibility for services determined?Response: The patient must have insurance or be able to pay out of pocket.

The company puts the patients requirement first and that is all what Liberty Dialysis is dedicated to offer to its clients. 11.
