In my training, I have been taught on how to handle the patient and pay the required attention to him/her. With the knowledge and skills that I have, it is my duty to provide essential care to any person in need and also provide more care in the field of practice. Assessing people's needs in making them manage their health is my great concern.

The portfolio will demonstrate what I have accomplished and done and what I am doing in preparation for the transition. In this portfolio, I will give details of my skills and knowledge, my attitudes and values, my preferences and interest, and action plans and goals that show how ready I am to start practicing as a Registered Nurse.

There are several things that I consider to be important in my life. The first aspect I consider in the course of my work is time. I take any single minute of my time to be a very important asset to me and to my clients or the patients I’m serving. I not only consider time to be important during my work but also in my personal life. 

An example is the Guyra community. It comprises of Guyra town dwellers and villagers in Ben Lomond, Black Mountain, Ebor, Llangothlin, and Tingha main villages. Guyra has located in the New England Region of New South Wales, Australia. It is the coolest part of Australia located on the New England Highway. 

Type 2 diabetes is a chronic condition affecting the way the body metabolizes sugar. When suffering from this condition, the human body will resist the effects of insulin or fail to produce enough of the sugar regulation hormone so that the body will not be able to maintain the required glucose level.

Almost everyone in life needs medication young and old alike. However, in this case, the cycle mainly rotates on registered nurses, the aged and the phenomenon of medication. The complexity of medication is usually determined by both the age and the severity of the disease. For instance, kids and the aged are regarded as a group that is vulnerable to disease as compared to youths and average people.

In the field of nursing, the safety and quality of work of nurses have been affected by various factors. This includes the heavy workload that the nurses are exposed to. The heavy workload has negative impacts on the quality and safety of the patient. However, it is also important to note that changes have taken place and have positively influenced nursing practices.

Chronic health conditions involve illnesses that cannot be cured but can be managed. The management of chronic health conditions like type 2 diabetes requires adequate information with looks at the specific condition.  The target of this plan is to provide a development need for a patient who is due to be discharged and is suffering from type 2 diabetes.

Quality healthcare delivery is an expansive umbrella that covers patient safety. In Sherwood and Barnsteiner’s (2012) view, quality defines the optimal balance between the likelihood of attaining excellence and attaining standardised values and norms. Quality is contingent on actors such as standards, or values and norms as the determinants of optimal possibilities.

One of the key factors in the provision of quality care and prevention of adverse patient outcomes is a nurse’s clinical reasoning ability. Clinical reasoning refers to the process by which nurses and other clinicians collect cues, process information, come to an understanding of a patient problem or situation, plan and implement interventions, evaluate outcomes, and reflect while learning from the process.

Racist ideas are often based on a feeling of alarm when confronted with people seen as being different in some way, but also include such things as perhaps having different ethical values, customs, accent, points of view and religious adherence, or are perceived to do so,  and produce negative attitudes.

Nurses who are registered are exceedingly sought after in Australia and in the whole world. This chapter introduces general knowledge that I need to know about becoming a registered nurse. Registered nurses have a role that is paced fast and challenging.  Registered nurses can make choices from a variety of career opportunities that are widely available.

People with renal failure require the highest standards of care in order to improve their standard of living. As such, nephrology nursing has evolved to become a distinct area of specialization in nursing. The complexity of renal diseases has also widened the scope of nephrology nursing to encompass several other distinct specialty areas.

It is essential to state that aged individuals and disabled members of society are faced with health challenges that may require special attention to meet most of their health needs. A number of such individuals are in designated facilities such as residential aged care facilities for elderly individuals.

Obesity is increasingly becoming a major public health problem in the world. It is associated with the accumulation of excessive fat that may impair health.  Obesity is responsible for the death of about 2.8 million people in the world every year. The number of obese people has been doubling over the years since the 1980s.

Healthcare systems are supposed to be effective, efficient, and quality to the community, users, and founders. Different health units employ different approaches to improving the standards and quality of healthcare services. Quality assurance in international organizations and countries employs professionals to oversee the entire process.

Nurses are faced with increased demands regarding making decisions about care on a daily basis. However, the process of making a choice which is the essence of decision making is a complex one. Based on O’Neil et al (2005, pp.66) the complexity of clinical decision-making calls for a broad knowledge base as well as access to reliable information sources while working in a supportive environment.

Notably, decision-making in nursing plays a critical role in ensuring that the right judgments are made with the objective of providing quality healthcare services. The professionals in nursing professionals are expected to make decisions that have a positive impact on the medical field. Judgment in nursing is considered to be a professional choice, but not a task in real-life practice.

Reflection is a teaching practice approach that enables the teacher to look back on how he delivered a session, make judgments and alter his teaching behaviour based on his evaluation (Valli, 1997, cited in Jay and Johnson, 2000). It is an important approach because it enables an individual to evaluate his strengths and weaknesses.

Communication is an essential tool that necessitates human interactions and helps simplify them. Different communication models are available for application in various settings, and they help explain the human communication process (Morgan, 2002). Inventions of different models rely on the concept of sending and receiving information and the parties involved.

It is essential to state that Mrs. Green is a 78-year-old woman who lives alone in a single-story unit. However, she has children and grandchildren who live away from her. Mrs. Green was alone when she experienced disturbing unrelieved central chest pain and left shoulder pain for about 10 minutes.

Decision-making in nursing ensures that nurses can improve the quality of care by improving the decision-making processes by using diverse methods, plans of care, and other theories. Decision-making in clinical practices is a significantly vital skill to ensure the nurses/ students perform their responsibilities in a systematic manner.

Evidence-based practice is the conscientious, explicit, and judicious use of current best evidence in making decisions about the case of individual patients (Baum, 2003). Such decisions include a choice of treatment, tests, or risk management for individual patients, as well as policy decisions for large groups and populations (Baum, 2003). 

The societal balance suggested by this influx of positive thinking can also be seen from a sociological perspective as being basically functionalist. The behavioral theory assumes that while the above is useful, it will not succeed unless healthy concepts are socially constructed and the environmental factors are reinforcing.

The legal, clinical, and ethical dimensions in nursing health care practices are all interrelated with one another in the sense that the ethical dimension of nursing health care practices serves as the universally accepted medical and health care professional practices in terms of moral issues.

Henry was a 30-year-old construction worker in apparently good health until he suffered a fall in his work. He was taken by ambulance to a busy, urban emergency room, was "triaged", initially evaluated by a resident physician, and placed in a cubical to await services. Generally, in the medical world, there are Clinical ethics and laws put in place to instill disciplines in the medical environment that safeguard the rights of both the patients and the medical practitioners. There are many laws relating to people practicing their right and freedom to make decisions.

The research assistant documented the duration of the consultation by use of a stopwatch, right from the time when the patient entered the consulting room of the advanced nurse and stopped when he or she left the room. The length recorded included the amount of time used for prescriptions and any interruptions that might have occurred. 

The code of ethics in nursing comprises certain guidelines based on which the nurses are required to conduct their responsibilities consistently. These generally provide values along with principles with the assistance of which the nurses can develop ethical relationships, behaviors, decision-making, and responsibilities. 

Under the patient’s rights provided under the Universal Declaration of Human Rights, physicians and hospital authorities need to ensure that they share a patient’s diagnosis with the patient as withholding this information can be deemed as a violation of these human rights (Freeman and Pathare 2005). 

The application of all of this is concentric upon the fact that policymakers within the government should be mindful of the fact that remuneration for the nursing profession should match the increase in standards and policies that are now being implemented; and have been implemented over the past several decades.

The independent prescriber was ultimately the doctor who diagnosed the problem of the client and made treatment plans and CMPs whereas the dependent prescriber had the means to take care of patients in the long run through clinical guidelines, and treatment plans that could be prescribed according to the patient’s needs, looking at his history.

We have reflected on their health problem of Rosie, which is her Asperger syndrome. Asperger Syndrome has been defined as a milder form of autism and related health, physical, mental, spiritual, social, and sexual aspects have been discussed and it has explained how even a mild shift in behavior can be a reason for someone to get diagnosed.  

Dementia is a general name for a variety of cognitive disorders whose symptoms include long-term memory impairment; difficulty in areas of language and object recognition; and disturbance of executive function. According to Cantley, dementia is the deterioration in brain performance to a level at which social and personal functions are affected. 

Despite the fact that COPD affects the lungs majorly, the disease can also create an overhaul of emotional challenges. It may cause limitation or one’s ability to be happy and jovial in life. In such cases, the use of technology usually inflicts additional pain and suffering and may result in unconstructive outcomes. 

Pacific healthcare is a health provider that is the largest in Santa Barbra County since it is a large institution with three hospitals inclusive of the pacific memorial hospital, pacific Cabrillo hospital, and Pacific Isla Vista. The healthcare provider also has two nursing homes and clinics that provide outpatient care to patients.

Dialysis is the ultimate lifetime treatment with nutritional and psychological counseling making important subsidiary clinical management. Dementia is a mental problem, mostly developed after 50 years of age. At times, dementia can be developed even before the age of 50. Loss of memory is the major symptom associated with dementia. There are different types of dementia such as Alzheimer’s disease, vascular dementia, diffuse Lewy body disease (LBD), and alcohol-induced dementia.  

The healthcare sector is among the most sensitive aspects of the United Kingdom’s economy because it helps in safeguarding the population from infections to enhance production. Various types of research on the effects of short staffing of nurses in the country on the safety of the patient and quality of healthcare indicate a direct correlation. 

It has the effort to provide care that is palliative adequately, and it will be a study of care. According to the 2015 Council of nursing and midwifery, a person’s confidentiality together with his or her family will be considered in the assignment.
