Mr. P’s Case Study Solution Solution The situation facing Mr. P and his wife is a complex issue due to the need to maintain diet restriction, managing his polypharmacy and the task of caregiver, which becomes difficult for his wife due to financial constraints and Mr. P’s depreciating health. The best strategy to approach Mr. P’s care is to advocate nutritional therapy as prescribed by the physician. This will keep his body strong and able to fight against other diseases that may attack the patient. The caregiver’s knowledge on the quantity of fluids and other dietary check will limit complications that lead to the patient hospitalization and increment in costs. The primary objective being treatment of the patient is important for the patient and his wife to comply with the prescribed medication with the help of an outpatient’s nurse supervision. The medications include ACE inhibitors and beta-blockers, which will improve the health of the patient by stabilizing the patient for more therapeutic measures (Escott-Stump, 2008).
This approach to care recommends a treatment plan that is cost effective, improves the psychological, physical, and prolongs the life of the patient. Most patients suffering from CHF diseases are adamant to undertake medication while in hospitals since they pray for their demise (Escott-Stump, 2008). Mr. P’s condition is similar, thus there is need for the patient to receive home care from his wife or the wife may hire a caregiver. The home care will involve contacting the nurse daily after referral and a weekly outpatient nurse’s visit to ensure the patient is in good condition (Meerabeau, & Wright, 2011). The patient and the caregiver should be educated on the various ways to improve the health of the patient and the consequences of failure to oblige with the medication plan. Psychological education is important to ensure that the patient and the caregiver learn to accept the disease. The caregiver will acknowledge the essence of keeping constant communication and trust with the patient that will reduce the pain experienced due to depression and despair. The patient will learn to become friendlier with the caregiver thus building a bond that will enable decision making easier for both parties and solve depression issues. The caregiver will learn the importance of trusting the patient on some decision making to enhance the patient feeling valuable.
The caregiver will also learn the importance of socializing. This will improve Mr. P’s wife health condition and enable her to make the best possible way to deal with the patient. Dietary and physical activity will enhance the patient’s health thus; it is wise for the caregiver to have a dietary list that will help monitor the feeding habits of the patient. The patient should avoid alcohol and nicotine due to the difficulties in breathing and his lungs condition. Engaging in physical activity enhances relaxation and improves the physical health of the patient thus reducing hospitalization and health care cost by more than 40%. Recording of the patient’s weight on his diary will help monitor the responses of the approach plan. The outpatient nurse’s supervision will help ensure improvement of the home care (Meerabeau, & Wright, 2011).
Assessing the patient’s and family health literacy will help the counselor to know the best strategy to educate the patient and family. The counselor will educate the patient and the caregiver on the importance of changing their lifestyles; manage fluid balance and the whereabouts of the implantable cardioverter defibrillators. The bond created between the patient, caregiver/family and the nurse will help the patient and family to become open to questions and after counseling the provision of written information to improve the patient’s health and the expectation and ways of dealing in case of death of the patient by the family (Meerabeau, & Wright, 2011).
