In addition, one should be a holder of a high school diploma, and has accomplished the conditions for graduation from an accepted program. In applying and taking the licensure exam, an individual should first apply to the department of health to take the examination. In applying for the examination, an individual should complete the application form and pay a fee set by the board, examination fee, plus an actual per applicant cost to the department for the purchase of the examination. In applying for the examination, one should provide adequate information to be submitted to the law enforcement department for verification of any criminal reports. In addition, an individual should prove that they are physically and mentally sound, and should submit their education credentials. After taking the examination, if the applicant passes and provides evidence of satisfying the academic conditions stipulated by the board, she or he shall be entitled a license as a registered professional nurse.
