Mechanical ventilation is commonly used in the intensive care setting to artificially ventilate persons who are unable to breathe spontaneously at all or are unable to provide themselves with adequate spontaneous ventilation to guarantee satisfactory gas exchange (Boles et al.,…

The main reason the patients on ventilator needs to be monitored and managed with dexterity is ventilator associated pneumonia (VAP), which adds to the morbidity and mortality statistics associated with ventilator management and hence this condition needs to be prevented. The current practice in the United States is to use the ventilator bundle as a nursing guideline for practice in the intensive care unit with focus remaining on the head of the bed elevation (Tolentino-DelosReyes et al., 2007). As has been indicated by Reeve and Cook (1999), VAP is the most serious complication of critical illness, and this occurs not due to the illness per se, but due to management of the patient. The magnitude of the problem will be more relevant from the fact that this accounts for almost half the intensive care unit infections in Europe (Vincent et al., 1995). A guideline for care in the current practice as a specialist nurse in a district hospital would be the best tool, and such tools can be accomplished through the evidence base. This would involve a way to improve current practice through the critical and thoughtful integration of best available evidence from research into clinical practice (Manthous et al., 1998). Research is a systematic enquiry on any subject, and therefore, research in the area of intensive care nursing relevant to people receiving ventilator breathing would mean a process to answer questions relevant to practice in this area (Cormack 2000).
This has an implication for the nurse involved in clinical practice, and it is required by the standards of practice that the nurse involved in practice expansion must ensure that the practice is evidence based and can be justified in the literature. Price (2001) states that a thorough literature review provides a foundation on which to base new knowledge and usually is conducted ...Download file to see next pagesRead More

Ventilator associated pneumonia

...? VAP Bundle and Implementation Ventilator Associated Pneumonia or VAP is one of the leading causes of mortality in patients who are ventilated. It is also the leading cause of death among hospital acquired infections. There is some evidence that implementation of certain interventions, grouped under VAP bundle, decreases the incidence of VAP from 20 per 1,000 ventilator days to 5 per 1,000 ventilator days (Safer Healthcare Now, 2011). The VAP bundle includes four components of care and they are: 1. Elevation of the head of the bed of the patient This is the most integral...

Evidence Based Approach to Ventilator Acquired Pneumonia

...? Evidence Based Approach to Ventilator Acquired Pneumonia Though medical interventions aim at restoration of patient’s well-being, some medical procedures may pose as demerits by acting as source infections. Ventilator acquired pneumonia (VAP) is one of those infections (Chastre & Fagon, 2008). It is a disease of the lung tissue occurring forty eight hours after procedures like tracheostomy or placement of an endotracheal tube to the patient is done. These procedures impair the integrity of the respiratory tree (CDC, 2012). Various journals have been published on other various VAP issues affecting its severity and outcome. These include...

...and radiographic examination. Antibiotics should be used judiciously to prevent resistance. Other preventive measures like respiratory therapy and basic nursing intervention should also be adopted to reduce the risk of VAP (Chastre and Fagon, 2008). Ventilator associated pneumonia is limited only to those patients receiving mechanical ventilation. The general understanding of VAP has significantly grown over the past year in terms of risk factors, pathogenesis, diagnostic testing, therapies and prevention. This article assists the clinicians to understand VAP in a better way especially in terms of...

...thereby reducing the incidence of VAP , mortality and morbidity rates, diminishing the length of hospital stay and hospital cost. Thus, we can conclude that if the preventive, diagnostic and treatment proposals outlined in the ventilator care bundle approach in this paper are practiced with efficacy, a definite and improved outcome for VAP affected ICU patients can be achieved. Ventilator-Associated Pneumonia Middle Range Theory Application establishing proper nursing care for patients. When properly applied, this theory could provide resourceful methods remedies and cautions against some of the challenges that are prevalent in the field of nursing. The theory is structured and explained in ways that could be relied upon to increase the efficiency in the functions of nursing as they apply to different situations and areas. The task of combating, containing, and preventing VAP is one that should involve multiple approaches. These approaches are generally designed to foster proper management of the nursing situations towards the general objectives of healing and the promotion of human dignity as spelt out in the underlying philosophy of nursing. Polices and guidelines There are several polices set according to the hospitals. However, the polices in each hospital are common. For example, in the hospitals the use of an ongoing program on the infection and preventive control on the patients undergoing mechanical ventilation should be adopted. Information such as the location of the local and epidemiological evidence based strategy needs evaluation and it should therefore include the information on the evidence-based...

The drug is less contaminated and is administered in tubes to patients at lower costs. (D) The radiograph method of analyzing the tracheal aspirate is not recommended as an accurate way of diagnosing ventilator-associated pneumonia (Jones, 2010). The best method is employ machine based therapy, which can be able to detect microbiological and microscopic entities in the secretions. The use of the non-invasive and invasive method is recommended for use. The method...

From this perspective, I want to find various methods of preventing ventilator-associated pneumonia in the ICU patients. In this paper, I have conducted a library search using the most current medicine journals about the occurrence of ventilator-associated pneumonia in ICU. Huang, S. J., Huang, H. J., Yu, S. ...of the sample size that was used could be able to be overcome through prolonging the length of the study to include a large sample or through making the study multicenter. As described by Hubmayr (2002), the duration of mechanical ventilation of antibiotic exposure before the onset of VAP are considered some of the most important factors in the microbiology, incidence, and severity. Conclusion The findings show that the study did not provide sufficient evidence to support changing the clinical practice. The trend that is obtained from the research shows that a reduction in the incidence of VAP through the increase of angle of head evaluation from 25 to 45. The fact...
