Patient Centered Medical Homes Lecturer: Patient Centered Medical Homes Purpose/Rationale Provision of quality health care services is a basic human right. Patient centre medical homes (PCMH) are innovative primary health care centre for the provision of quality services. It is a foundation for accessing efficient and cheaper health care services. The primary goal of PCMH is to transform the centre in order to deliver effective primary care services at affordable prices. The program offer practices for care organization for patients, nurses working in groups and coordination as well as tracking patient information. This institution partners with personal doctors and patients when required and exchange health information. They can also assure patients high quality services in an appropriate way. The primary purpose or the objective of the research proposal is to analyze the way PCMH would transform primary care; thus offering effective access to affordable and high quality health care services. Literature Background The literature study indicates that every human being has a right to access to affordable and high quality health care services. Therefore, it is the role of the government to ensure that citizens access to quality services at cheaper prices. Feldman and Alexander (2012) argue that nurses should take actions and put efforts in their roles to provide quality patient care. They should take the leadership roles in delivering effective services. This is essential because it will enable them to earn respect from their patients. According to Feldman and Alexander (2012, p. 33), leaders in the health care should communicate effectively in the health care management. This is because communication is essential in solving social conflicts and conflicts that arise between patients and nurse over payment issues. This will contribute to successful performance; thus, enabling nurses to deliver valuable services that meet the demanding needs of patients. Research Question: 1. How would PCMH transform the primary care in order to deliver quality and affordable health care services to the public? 2. Will transformation of the PCMH health care benefit all citizens? Research Plan/Procedure As per the objective or primary purpose of the research study, I will employ systematic procedure that will meet the objectives of the research. This will be explained in the kea area of focus, which includes: Research design Data analysis Data collection methods Strategic qualitative approaches Research Design The research will employ qualitative study in order to provide empirical investigation of social phenomena through the use of different data collection methods. Therefore, the research hypothesis will tested in order to determine whether the transformation of PCMH will contribute to the provision of affordable and quality primary care services to all patients. Under qualitative study, the researcher will test the hypothesis. This is through collecting and measuring the incidences of numerous views or opinions provided from the chosen data collection methods. The qualitative research is effective because it quantifies data and provides generalized findings from the sampled populations of concern. Data collection will be structured in the form of questionnaires, use of random sampling and field surveys. First, the researcher will structure questionnaires and distribute them to patients and nurses working in the PCMH. The research will be carried out from morning till afternoon hours. The questionnaires will be structured in the form of simple questions in order to help the researcher to gather adequate information from the interested respondents. Questionnaires are effective because they do not require many efforts and they would enable the researcher to compile and analyze data effectively. Secondly, the researcher will use random sampling from the interested population to represent the whole population. Random sampling will help the researcher to control the required study. Lastly, the researcher will employ survey method, and he or she will ask patients in case they receive effective services. The researcher can also request them to provide their opinions in case they think that transformation of PCMH will enable them to meet their expectations. Data Analysis Data analysis will be descriptive in nature because the researcher will gather information and evaluate the results in descriptive way. The research will interpret the findings and generalize the information with the help of tabulated tables. The proven hypothesis will be tested for nudity; thus seeking to ascertain the significance of PCMH in terms of transformation of the primary care to provide affordable and quality services. The researcher will interpret all the research findings through the use of statistical analysis in order to construe statistics. Limitations Ethical issues may arise especially when employing varied data collection methods. First, the research may encounter ethical problem because not all respondents will be ready to express their view or opinions. This is because they may feel that the information they will provide would be disclosed to the third party. Thus, it is essential for the researcher to promise that the information they will reveal would be confidential. The study limitation may results because some methods such as survey and sampling will contribute to biasness especially when the researcher is collecting information. This is because not every respondent wild be ready to provide true information. The research will also oversimplify the findings because of different information provided from the research data collection methods. Data Collection Methods I will employ both primary and secondary data methods for collecting data. These methods include: a. Primary Data The researcher will make use of primary data to gather information. The primary sources for data collection may include survey, observation, questionnaires, experiments and structured interviews. This data involves collecting information from first-hand experience about the provision of health care services in the PCMH. It is advantageous because it is reliable and has high authenticity because the data is not biased because the researcher uses naked eyes to collect information. The researcher wills also avoid providing misleading information, and reliability is true when tested. Therefore, the information collected cannot be changed easily. It is also effective because of validity; thus, the researcher provides quality information, which is trustworthy. Therefore, the research will improve the validity, and it will yield effective information from the targeted population. However, the sources of data may be difficult to obtain when using this research type. This is due to lack of collaboration or population scarcity, and it is costly. a. Secondary Data The researcher will employ secondary data, and this involves the information collected from the available sources. This data type is inexpensive and easily obtainable because it is accessible or obtainable than primary data. The sources of data for secondary include printed sources such as journals, book or magazines, electronic sources, unpublished personal records and government records. This type is advantageous because it is economical and time saving. It advances or makes one understand the research study predicament. It also enables the researcher to compare data collected from varied sources. However, it has some drawbacks and one of them is that data may be obsolete or out-of-date. The level of accuracy is low because it involves analyzing statistical data; thus contributes to time consuming. Strategies for Qualitative Approaches i. Grounded Theory Grounded theory is an effective technique employed in research activities, and it involves finding of theory through analyzing data. This approach operates in the reverse way because instead of beginning with the hypothesis, the research collects data through the use of numerous methods (Creswell, 2009). This theory aims to generate or discover a certain theory. Therefore, this method involves selecting the participants through the use of purposeful sampling techniques. The purposeful sampling is essential; thus when choosing the participants, one sample case or people with unique characteristics. One may also use comprehensive strategies and this involves selecting varied cases or targeted individuals. Moreover, grounded theory generates information that includes both deductive and inductive thoughts. The main aim is to formulate hypotheses that are based on theoretical notion. The goal of using grounded theory is to discover the way participant are concerned with the reformation of PCMH and find the way forward. Therefore, I will use semi-structured interview and questionnaires to obtain information. The questions in this study will repeatedly ask what role the organization is playing to transform the healthcare and what are the main issue affecting the patients. Grounded does not aim to find the truth but conceptualize things that are taking place in order to solve the problem. However, one will not generate hypothesis in advance because of the preconceived hypothesis in the grounded theory. ii. Phenomenological Research This is a body of knowledge and it expresses information mathematically without taking into considerations detailed attention to the fundamental importance of the phenomena. This theoretical study interprets and analyzes experiences of participants in the field in order to determine their perspectives. This theory is based on the notion that there are numerous ways of analyzing and interpreting the similar experience. Therefore, the meaning offered about the experience is what constitutes the reality of the research. In this theory, I will select participants in the PCMH in the organization because they have been working or they have worked for a long time in the organization; thus, they may have experience about the research problem. The participants can also articulate their emotions; thus, it is significant to articulate their emotion and share experiences. In this qualitative approach, I will use structured and semi-structured interviews in obtaining data results. However, I will analyses data using this theory description of the initial experience of the researcher with the experiences. iii. Ethnographic This is a research approach that aims to explore the cultural phenomena. The field study reveals the experience and meaning of system in the cultural group of people in the study area. This theory includes providing empirical information for different cultures, and it involves presenting ideas graphically (Creswell, 2009). This study is holistic in nature; thus, it should include a brief narrative, and the cases offer substantial contributions towards the study phenomena. The data is obtained through observation, and this may involve use of field notes. Another way is through using field notes and record for describing or reflecting upon the descriptions. Interview and document analysis are employed in the research. This is essential because interviews and observations enable the researcher to store data in a large amount of narrative data. iv. Case Studies Case studies are effective qualitative approaches for research analysis. There are varied types of case studies and among them include historical organization, situation analysis, observational cases, oral history and multi-cases. This approach requires one to focus in-depth the problem under study and understand it. It employs a single main question but with numerous sub-questions. Therefore, I will select participants, time and identify the site effective for research purposes. I this same, I will obtain data through observation method. This involves use of field and record information for observations. It also employs the use of interviews, which should be structured in nature and begin with a general opinion. Feldman and Alexander (2012) reveal the way nurses need to practice leadership roles effectively, and this is through employing varied research techniques for achieving the stated objectives of an organization. v. Narrative Research Narrative approach is an effective strategy for analyzing and interpreting study events. It is a powerful technique for transferring and sharing knowledge and one should be bound to effective cognitive memory. The narrative is structured in the form of tales or perceptions determining the way PCMH would be transformed. The narrative research should be creative, precise, liminality, and it should engage the audience. Creswell (2009) argues that narrative research the researcher should narrate effectively in the form of historical narrative. The data analysis employed in reporting involves the use of interviews and questionnaires, but the researcher should make use of themes in the narrative research study. Conclusion The research provided the rationale or the purpose of research study. Therefore, the primary purpose of the research proposal was to analyze the way PCMH would transform primary care in order to offer effective access to affordable and high quality health care services. It tackled on the literature review and what other researchers have provided about transformation of health care services. The research also employed different research design methods including use of questionnaires, survey and random sampling. It provided the hypothesis of the study that was to be tested and proven correct. Data analysis was descriptive in nature, and the researcher gathered information and evaluated the results with the help of statistical data. The researcher used different strategies employed in qualitative research approaches for data collection. Lastly, the conclusion summarized the proposal and the research findings.
