Let’s take the example of Finketstein, et. al. (1997) on the case of physicians. In their study, they reported that physician can harm the patient by a medical error. It was stressed out that patients should be informed of the errors that were made during the duration of medical care. More often than not, the full disclosure of medical errors has been forgotten because of some psychological and social considerations of the physician. “Physicians have been given little guidance regarding how to conduct a relationship with the patient after such injury.

 We argue that the physician must continue to respect the patient, and communicate honestly with him or her throughout their relationship, even after the patient has been injured.” Apart from the situation abovementioned, David reported that there are other barriers to an error-free medical practice: blame culture, lack of leadership, lack of peer-review protection; and the absence of a collaborative voluntary national reporting system. By overcoming these barriers, medical management would improve resulting in better medical service that is efficient and ethical.

To reduce the occurrence of medical errors in order to improve safeguarding the patient’s safety, IOM recommended that stakeholders should take action which includes a comprehensive examination of problems with the system and safety strategies and policies needed for prevention.
