Quality Improvement Plan The North Park Health and Rehabilitation center is located in Texas in the United s and is found in McKinney. They are dedicated in the provision of health services to patients registered with both the Medicare and the Medicaid providers and more so it accepts private payments. Other kinds of payment such as the veteran’s benefits, long term care insurance are also acceptable. With a bed capacity of more than one hundred and forty, the nursing home provides several health care services. It provides care to those requiring rehabilitative, long term or secure dementia assistance. Basically, it provides medical nursing care to seniors with serious illness or disabilities. The Services Offered To accomplish this vital role in the medical care, it incorporates various services. These services include: Laboratory services, Radiography services, dental, dietary and housekeeping services. In an attempt to assist the elderly, it has established the mental health, nursing services, occupational therapy among other activity services. Additionally, other services such as the pharmacy, physical therapy and physician services are also available in the rehab center. Towards giving back to the society, the rehab has established social work services. The mission of the health centre is “assisting you and your aging parents”. The health centre has established services and a recipe that is geared towards ensuring the welfare of the elderly. It also aims at new lifestyles for seniors; through their pursuit for better health and their specialization on the elderly, this has ensured that the organization improves the health care for the senior citizens. Having undergone a certification under the international Standards Organization (ISO 9001:2000), the health centre has developed quality Improvement Plan in compliance with the certification. The qualitative improvement plan is well highlighted within the organization and includes the development of the organization’s quality policy and a quality model. Further, the health center has the duty to undergo the certification audit and therefore make required corrections. Additionally, the staff is always educated on the quality management systems that have already been developed by the organization. The center on the other hand has established and therefore ensured the implementation of the whole laid down process for fulfilling the quality improvement plan. The education of the staff on ISO 9001:2000 standard, scope of work and organizational impact have been ensured in the rehab hence ensuring that the human resource is up to its task. Mission The mission of North Park Health and Rehab is to provide quality health care for those who are in need of skilled rehabilitative and nursing care, secure dementia care and long-term care. The goal of this quality improvement plan is provide care that is effective, safe, equitable, patient centered, timely and efficient. To achieve the goals of this quality improvement plan, all employees of North Park Health and Rehab will take part in the systematic and continuous quality improvement efforts. Our QI efforts will be directed at improving care delivery and their support processes with an aim of improving patient outcomes and high quality business operations. This is mad possible through the analysis of variance, peer review, the review of clinical results, performance appraisal and other applicable QI techniques. Role of Consumers The QI process that could greatly help this healthcare organization is focused on six areas: safety, effectiveness, efficiency, equitability, timeliness and it should also be patient centered. When the QI process is focused on the patients, it gives them the freedom to contribute to the process without being inhabited by any circumstance. One of the role of consumers, or patients, is to provide feedback on what they think about the services provided by North Park Health and Rehab. Since it is the patients who are supposed to gain from the services provided by this organization, they should be allowed to give their views about what they think about the services. External quality indicators for Consumers Some of the external quality indicators that are available to consumers include internet reports by authorized medical bodies. These reports normally contain information about the kind of services offered by different health organizations and ratings of the same. These ratings are reliable since they are normally done by highly qualified professionals (McLaughlin and Kaluzny, 2006) . Another external indicator of quality is in what people say about the services offered by North Park Health and Rehab. When a facility offers good services, people will talk about it. Hey will talk too when the services are not up the required standards. If there are some incidences that the healthcare facility does not take care of like it is supposed to, the consumers will know and they will talk bout it. The public opinion about the facility will make a big impact on how information about the quality of services flows. The number of insurance companies represented are also an external indicator of quality in the facility. Insurance companies worth their names normally are associated with health facilities that offer exemplary services. In this case, North Park Health and Rehab has working relationships with high end insurance companies which means that the quality of services is quite laudable. Utilization of these Indicators by Consumers in QI For there to be maximum quality improvement, the QI planner needs to use as much information as possible. One of the most important sources of this crucial information is from consumers, and in this case, the consumers are the patients. From the rankings that are found in medical reports, public talk and associated insurance companies, the consumers will have enough information to make some valid contributions for quality improvement. Utilizing Stakeholder Feedback in the QI Process The QI process should be inclusive of everyone affected by the services offered by the North Park Health and Rehab Center. This is why it is important not to ignore any stakeholder feedback, however trivial it might seem, in the QI process (McLaughlin and Kaluzny, 2006) . The employees, patients and owners all have an equal opportunity of contributing towards the well being of the health facility. This includes contributing any ideas that might be helpful in improving the quality of the services offered by the facility. All these stakeholders have vested interests in the center; therefore, their contributions should be taken seriously in order to build a stronger healthcare facility for the people of North Park. Conclusion The mission statement and consumer policy of North Park Health and Rehab Center are all supportive of the provision of high quality healthcare services to anyone who might need them. The Quality improvement plan will among other things include the activities stated below: Credentialing of professional staff Satisfaction survey among physicians, staff and patients Medication therapy Medication errors Mortality/morbidity review Risk management activities Management of utilities Several methods can be applied to achieve quality improvement.
