Institute Organizational theory: implementation of change of moving to a “patient-and family- centered care model” This model is and innovative approach aimed at delivery, evaluation and planning of health care based on mutual partnerships among families, healthcare providers and the patients. The pillars for the patient and the family centered care model are dignity and respect, participation, collaboration and the sharing of information. The model matters and is important thus should be taken keenly by all and should be implemented in all departments.

The key to implementation of this model is the joining and working together of different departments with one aim which is moving toward the patient and family care model (Abraham, 2012, p. 45) For the best implementation of change towards this model, first, there should be personal and team education, this would help in the proper understanding of the challenges presented and the possible methods to deal with these challenges, for this to go as required the process must start with a self-assessment test.

They should then highlight the concepts and the approaches, and then conduct the training sessions (Moretz, 2012, p. 107) The second step towards moving to this model is educating others, in that those who understand the concepts of this model and have enough knowledge regarding strategies and programs can share this information with other staff members and their colleagues. As the information spreads, even the families come to join the process. This may be through professional and personal stories, organizational leadership staff and even peer mentors The third step for the implementation of the model is the development of infrastructure, this helps in sustaining and creating improvements in many institutions.
