He has chronic asthma and has been admitted to the local hospital on several occasions with exacerbations of his asthma. Following his last hospital admission Jack was referred to the community health centre and has an appointment with the community nurse in the community based asthma education program – 4) The community nurse undertakes an initial interview. He is prescribed both preventer and reliever inhalers to treat his asthma. Jack says he has been increasingly worried about his symptoms of asthma as he wakes overnight with frequent coughing and his concentration at work has been affected by his poor sleep. He also reports that he gets very breathless when walking up hills and to the train on his way to work. On the basis of this information the Community nurse plans the following interventions: a) Tell Jack about asthma. b) Assess Jack’s baseline respiratory status including rate, rhythm and depth of breathing, chest movement, use of accessory muscles, breath sounds and peak flow. c) Encourage Jack to drink warm fluids. d) Ask Jack to demonstrate how he uses inhalers. e) Encourage Jack to sleep in a semi-Fowler’s position to increase O2 saturation; promote lung expansion and improve oxygenation. f) Teach deep breathing and relaxation techniques to relieve tension and ease respirations. g) Visit Jack weekly. 6) Explain how effective this plan will be in assisting Jack self-manage his asthma.
