Transition involves both the process and the outcome of complex person-environment interactions, which may affect more than an individual and is embedded in the context and situation. Change is a period in between stable states and squarely falls within the domain of nursing when they relate to health and illness or perhaps the reactions to the transition are exhibited in health related behaviours (Tellez & Seago, 2013). Transition acts as a dependent variable while the health illness condition is the independent variable such as an acutely hospitalized ill patient or other situations having an impending state of transition. In addition, immigration may turn change into an independent variable, which may expose human to health challenges and influence responses to health problems as well as types of action to be taken (Kim, 2010). Consensuses regarding nursing domain revolves around human beings, environment and health are of great concern to the scholars and researchers from various disciplines. It is imperative to note that the nursing’s anxiety depend on their interrelatedness as the intricate minimum from which knowledge for nursing practice evolves. The triad acts as the basic for nursing knowledge and therefore an additional concept may be vital in making up nursing. Transition is amongst the concepts that offer a key to interpreting person environment interactions due to the real and potential effects on health (Tellez & Seago, 2013). Transition as a process and an outcome results into a prescriptive theory that may be used to generate impact on practice and the nursing profession is gaining from conceptualization of transition. Meleis (2011), asserts that characteristic of change is that it is fundamentally positive and its completion means that an individual has reached a greater stability level in relation to previous events. Generally, change possesses process, disconnectedness, perception and patterns of response as its characteristics. As a process, transition must always have a beginning and the end where an ending is followed by a period of confusion and distress and eventually a new beginning. On the other hand, disconnectedness serves as the most pervasive characteristic that links with the disruptions relating to human feelings of security as well as loss of familiar reference points (Kim, 2010). The strangeness that is experienced between the past understanding and the perceived present as well as the inconsistency existing between needs ad availability. Transition may also be perceived differently amongst various communities and societies and this play an integral role in deciding the outcome. Moreover, transition should be understood as a personal phenomenon and therefore its outcome is linked with the definition or redefinition of self and situation (Meles, 2010). Looking at the dimensions of change, it is undeniable that transition cannot be experienced uniformly although there are some commonalities.
