Introduction
Background of Refugee Health Community Nursing Program.
Over the past five or so years, more than 62,000 persons have come in Australia through Humanitarian programs initiated by Commonwealth and with over 28 percent settling in Victoria. from (The City of the Greater Dandenong (2008). Public Health Plan for the Municipal 2007-2010, The City of Greater Dandenong). This program includes people who arrive under the umbrella of refugee programs and others on special humanitarian grounds. In general people who arrive under humanitarian programs have faced common catastropheres.Among them include suffering from gross violation of violation of human rights and persecution, longer periods of deprivation ,loss of their loved ones .most of the refugees come from war torn countries (for example Sudan, Iraq, Bosnia among others ) and as a result most of the arrivals in Australia have high mental and physical needs so they require extra social and health support services to be addressed.
Alice is one of the many refugees that have been heavily traumatized due to the experiences they faced in their home countries that forced them to leave and look for settlement elsewhere and she has found herself in Australia. Because of the bad experiences she has undergone she finds settling in Australia very challenging despite the numerous suites of services that the Australian Government has put in place the meet the refugees needs which are normally complex and diverse. The government of Australia provides these humanitarian services through community agencies and the objective is to make the refugees rebuild their lives and progressively become members of the Australian community who are fully functioning.
Integrated Strategy.
Integrated Humanitarian Settlement Agency (IHSS) is the government recognized body that is responsible for the provision of settlement support for the refugees who have just arrived. IHSS main aim is to ensure that the refugees gain self sufficiency as fast as possible by offering them special needs on case by case basis. The case approach has been found to be the most effective way to identify and address their particular settlement issues .IHSS has a well established frame work to which enables the entrants get access to the mainstream services. IHSS programs are normally six months but in some cases may be extended depending on the vulnerability of the client. IHSS limit their services; they do not offer services to refugees who arrive through onshore or even to the ones who have bridging visas or protection visas. The following are the services under IHSS;
Accommodation services -They assist in providing basic household goods in order to help them establish a household in Australia. They also source for affordable and appropriate accommodation services.
Counseling services for trauma and torture- these way they are able to assess the needs of the client and construct a case plan, referral for trauma and torture counseling.
Alice’s issue
She is severely traumatized due the loss of her husband and two of her children during the civil war in her home country and the miscarriage she had prior to leaving Sudan that resulted from violent sexual assaults within the refugee camp.
She is jobless and none skilled.
She is semi literate
She is struggling to find the food that she is familiar with.
Her son is not happy with his new school
She has accommodation problem, Alice and her son stay with her cousin who has 18 month old twins in a two bedroom house.
She relies on government allowances as her only source of income and is very insufficient due to the rising cost of living
She also fears suffering gastro-enteritis since her family members has the history of undergoing this problem during summer.
Discussion.
Having held this passionate discussion with Alice, the issues she has outlined are very serious and needs urgent intervention. Alice has a crisis even after getting out of another .Her case is a complex one I would suggest she seeks CCS –Complex Case support services since it an organization that handles refugees with complex cases.CCS is funded by DIAC and is instrumental on giving intensive, specialized case management services to refugees who have just arrived. As a second opinion on this case is to her to the offices of The Forum of Australian Services for Survivors’ of torture and Trauma. This agency assist refugees get over from the bad ordeals they have experienced and start building a new life in Australia. The organization has experienced professional counselors who will arrange for her therapeutically counseling and organize group activities for Alice.( The Department of Immigration (2008). “The Composition of the Offshore Resettlement Program." On accommodation the DIAC funded Settlement Grants Program (SGP) has the capacity to organize for her accommodation to suites her needs for settlement, for example her they stay 40 km away from where her son attends school, making him be unhappy with the school. SPG will consider this and look for her alternative accommodation. Also the fact that they are staying in a house that are small babies with her son is very inconveniencing to their cousin and she definitely feels the pressure of having to cope up with the sudden, unplanned increase of the budget since she the one who is paying all the bills and has just given birth.
For victims like Alice any help extended to them should ensure that the following are realized:
Increase her access to primary health services by making available social and culturally relevant services in Community Health Services in regions where there is high density of refugee population and settlements.
• the response of the health services to Alice’s needs by creating agency capacity under contact with Alice and others like her, liaison and links with other willing RHNs, development of the expertise not to forget the professional development, advocacy with other available providers.
• Enable Alice, her family and refugee groups have her/their health and wellbeing achieved through health advocacy and promotion, prevention and development of referral networking and collaborative relations with available general same field practitioners, much needed social support, orientation services and programs.
Primary health care scenario and activities made available to Alice should include:
Thorough health and unbiased social assessments.
Professional and sufficient nursing rationale and care.
Relevant crisis and emergency management advice and tips.
Careful, relevant incidental and special counseling.
Active socio-economic support and non-partial advocacy.
First aid relevant to her surrounding environment.
Medical management, which should include eventual monitoring and scheduled follow-ups.
Recommendations.
Alice should also be provided with maternity and the very important maternal and infant/child health services. There are two categories of health services which frequently provide care to newly and vulnerable arrived women like her, their children and desperate families.
As noted earlier Alice posses complex and serious health issue, which are social instances and conditions that often their needs are not met by the existing health service infrastructure? One of the inconveniencies is that the existing health infrastructures only provide parallel and silo like help and services to people with these kinds of issues, such as gastro-enteritis over summer. Many of the people like Alice assists require health care and attention from a various providers for different and complex conditions and possibly at different clinics and this can be done as a referral by a nurse.
Undertaking health promotion activities and related events during individual consultations and within groups and in partnership with staff from local government and other services. Various examples include: taking her to group shopping tours and walks of local supermarkets with your area dietician, discussion of diets with her and family, referral of Alice and others to the relevant institutions. Well- Women’s/ladies clinic for screening; health education to Alice and like minded groups on requested tissues such as contraception.
In the case of transport problems, there exist many very important reasons why a nurse should wish to assist a client with transport maybe to a health service or other important service appointment. These are usually closely related to either the ultimate needs of the client which in this case is Alice, ensuring that Alice attends her appointments .The nurse will mostly sometimes personally transport Alice to any appointment if: a particularly important consultation; As the nurse wants to make sure the Alice attends given the relevance and importance of the consultation and/or procedure for long and short term health status of the Alice; the nurse wants to personally relay and communicate confidential or very sensitive information to the health practitioner or professional; there are articulate and particular public health concerns associated with the Alice’s attendance; the nurse needs to perform some preamble/ activity with Alice prior to or after the appointment (for example pick up some prescription, medication or health education); the nurse wishes to assess the home surrounding, environment or other family members history and if the Alice needs special reassurance or assistance.
