The OSHA Form 101 is an individual incident report that provides added detail about each individual recordable injury or illness. A suitable insurance or workers’ compensation form that provides the same details may be substituted for OSHA Form 101. Each employer, regardless of the number of employees or industry category, must report to the nearest OSHA office within 8 hours of an accident that results in one or more fatalities or hospitalization of three or more employees.
