Client Project Part II: Cardiorespiratory Fitness Assessment and Aerobic Exercise Program Age: 59 years old Gender: Male Height: 6’0’’ Body weight: 210 pound Resting BP and HR: Client has high blood pressure related problem and the do not have any heart rate problem. The classification of abnormal has not been detected on the heart beat rate, but it has been detected on the pulse rate or in the blood pressure condition. HR Max – Depending on the age group, the patient should have the heart rate from 80 to 136. As the client is a diabetic patient in nature, he is having high blood pressure, so it can be stated that the patient is vulnerable to heart dieses, brain stroke and other dieses. Special Considerations – The patients is having obesity and he does not have any other serious health related issues. It has been identified that the patient did not have any injuries and others illness apart from diabetes and high blood pressure. The patient is interested in doing physical exercise. The client has joined the gym for maintaining physical fitness and a check on obesity. Cardiorespiratory Fitness Assessment After reviewing the client’s profile, the Graded Exercise Testing protocol has been suggested for maintaining the health issues. As the patient is having obesity, so the physician has decided to suggest physical training facilities. It has been expected that the physical exercise will be suitable for restoring the patient’s condition. As the patient is having high blood pressure and diabetes, the clinical exercise testing is preferably suitable for the patient’s health condition. 1. Importance of the Cardiorespiratory Testing and High Cardiorespiratory Fitness The cardiorespiratory testing is considered to be one of the important diagnoses for those patients who are suggested for Clinical Exercise Testing. Cardiorespiratory testing is important for those patients affected by hypertension or high blood pressure, diabetes, smoking and obesity. Thus, the client is suitable for the cardiorespiratory testing. 2. Reasons of Selecting Specific Protocol And Mode Bicycling and trade mill exercise is suitable for the patient, as it is helpful for reducing excessive fats from the client’s body. Trade mill exercise is advantageous for patients having risk of heart diseases for disorders like obesity, diabetic and high blood pressures. 3. The trade mill exercise is considered to be a part of the field test. 4. GXT test includes HR rates, BP level and physical description such as height and weight of the clients. The test diagnoses the vulnerability of the heart dieses. GXT termination is helpful to identify the VO2max of the client by the exercised prescription. The field was mainly selected for the client, as he is a diabetic patients as well as having high blood pressure. 5. The HR and BP of the patients are 60 BPM and 120/80 respectively. Part III. Muscular Strength The 1 RM (repetition maximum) is the test of gold standard test which is concerning with the group mussels. For the upper body, the 1 RM is 300 lb barbell weight. The 1 RM upper body is 80% of 1 RM x 300 lbs barbell = 240 lb barbell. 1 RM lower is 80% of 1 RM x 300 lbs barbell = 240 lb barbell. Determine the Relative Strength and Determine the Appropriate Percentile Rank Adult patients are facing several changes on their health. The changes majorly affect the psychology of the patients. Adults of age group within 60 face problems related to obesity, blood pressure, blood sugar or diabetics and hypertension. The heath condition of the client is estimated based on the strength and weakness of the client’s profile. The client is of 60 years of age and does not have any major injury or major health related issues. The height of the patients is ascertained to be 6’0”. Though the client is little obese, but he does not want to make so much effort to lose the weight. Additionally, the patient is having regular exercise, which may have a good impact on the heath of the patient. The weakness of the client is that he is overweight. The client is having hypertension as well as diabetes. The client need to be cautious regarding weight loss and controlling his blood pressure and diabetics for maintaining a good health standard. Part IV. Body Composition BMI stands for Body Mass Index, which is mainly calculated by taking consideration of weight and height of a person. BMI is the one of the most reliable indicators of body fatness. % BF is calculated by including the four skinfold thickness measurement (SKF) and the single frequency ‘bioelectrical independence analysis’ (BIA). Skinfold Method- As the clients is having obesity, so the percentage of body fat equation method has been chosen in this case. As the client is an older adult male candidate of 60 years old, the equation formula is Body density = (1.1093800 - 0.0008267*sum + 0.0000016*sum - 0.0002574*age) = (1.1093800 - 0.0008267 x 110 + 0.0000016 x 110 - 0.0002574 x 59) = 0.866577 The body fat of the client is = (0.29288 x 110) – (0.0005 x 12100) + (0.15845 x 59) – 5.76377, where the skinfold sites (measured in mm) are abdominal, triceps, thigh and suprailiac. = 29.75158 BMI is determined by including body weight and height. The client’s BMI is = Weight (lb) / (Height (in) x Height (in)) x 703 = 210/ (72 x 72) x 703 = 28.48 The waist circumference is the method of measuring the waist size. The waist circumference of the client is 40 inches. The hip circumference of the client is estimated to be 30 inches Thus, the ration of waist to hip circumference is 3:4. Respectively, the client is needed to be scrutinized effectively with tests and accordingly, should be prescribed with certain exercise regime. BIA method- The Bio-electrical Impedance Analysis is a method of assessing the person’s body composition. The percentage of weight extra is 10 pound. The BMI method of identifying the body mass is comparatively less accurate than the approach of the BIA method. The BIA method is a safe approach, which provided accurate and reliable result of screening. The BIA approach is cost effective. The BMI approach is not able to provide accurate result of the patients’ health. Part V. Flexibility & Balance The two flexibility tests that can be conducted include star excursion test and unipedal stance test. For the mentioned flexibility, test of star excursion for balance assessment of any individuals, knee and ankle joints along with muscles of the legs will need to be assessed. Correspondingly, for the execution of the unipedal stance test, joints and calf muscles will need to be assessed. The normalized score for the star excursion test can be in the following form. The normalized score has been calculated taking into consideration the values of excursion distance of the patients along with length of his/her leg. [excursion distance (cm) / leg length (cm)] x 100 Note: Excursion distance measured for the patient is 6 cm, which can be regarded as a standard or normal value while the leg length is 86 cm. = 6/ 86 * 100 = 6.97 The normalized value for the test is around 6.97. This can further depict higher flexibility for the patient. Normalized score of the patient has also been calculated for unipedal stance test under two given situation with eyes open and eye closed. The value of normalized score for the closed eye intervention 44.7 on an average, while for the closed eye intervention the mean value is 15.2 for the three trials conducted. Part VI. Aerobic Exercise Aerobic exercise was selected for the patient, as it involves physical activities that can be considered for an obese physique. The initial exercise in this regard would be walking for ensuring proper warm up of the body.
