It addresses an overall strategy and vision for primary care. It also offers a framework based on values for patients to sustain and improve primary care practice. It seeks to improve the ability of primary care to play its essential role (Porter, Pabo & Lee, 2013).

Findings in article According to Porter et al. (2013), primary care is widely recognised as essential to any health care system (p 3). The findings show that many primary care practices are starved for investment. This implies that patients have a hard task finding primary care practitioners and are often disappointed with the ability of primary caregivers to meet their needs. Most primary care practices try to meet the desperate needs of heterogeneous patients with a single one size to fit all organizational approach.

This stresses patients as well as clinicians who attempt to serve them (Porter et al. 2013). The findings have proved that the only way to improve the quality in primary care is to recognize the primary care is the wrong way of analysis. Implementation of a health care plan means that it has to be deconstructed into different groups of services to meet different needs of people. These changes will lead to better ways of measuring outcomes and cost, new mixes of skills, new ways of accessing patients, new payment models and new approaches of integrating.

All in all, this seeks to offer value in primary care. The intent of the alterations made is to improve health outcomes and to do so with increasing efficiency. The value is often improved by multidisciplinary of clinicians to meet the major needs of their patients.
