According to OGP report (2011), process safety is a disciplined structure for controlling the operational integrity of systems and procedures tackling hazardous substances. It is achievable through proper applications of good design strategies, engineering, and maintenance and operating practices. The process safety strategy handles or deals with control and prevention of incidents that have possibilities of discharging hazardous substances and energy. There are major accidents that have been linked to poor process safety in industries handling hazardous substances and thus the need to analyze process safety risk management systems.

As illustrated above process safety is a sensitive matter in chemical and oil and gas industry. This has led to many authorities and bodies functioning jointly to ensure a common objective achieved. Department of Energy & Climate Change (DECC) operates jointly with other health and safety occupational bodies within the Downstream Oil Industry environment to consider the manner to address the challenges being experienced in the industry. Petroleum products are hazardous and require proper process risk assessment (Pitblado 1995).

The downstream segment of oil and gas industry touches consumers via products like lubricants, gasoline or petrol, jet fuel, kerosene, waxes, asphalt, diesel oil, fuel oils, heating oil, natural gas, and LPG (liquefied petroleum gas) as well as a number of petrochemicals (ICBNU 2004). This review is based on three segments with the first part concerns the introduction of key terms that are essential in process safety assessment system (Kletz 1999; Gustafson 2006).
