Health Center Unit Project Plan in New Jersey Introduction The medical unit to be started is not for profit organization with its mission of providing different medical services, creating awareness and sensitizing the community on teen pregnancy. It will be based in Paterson, New Jersey. It will be located at Paterson Health Center although it will operate differently in order to execute the goals independently. The new unit is part of the largest health care system, Metro Hospital Network. It is aimed at providing the social services to the larger population due to the demand in the country. The unit is to specialize in teen pregnancy, which has become one of the major problems facing the increasing population. In order to achieve the mission the health care unit has objected different objectives such as creating awareness, treatment, offering clinical services as explained later in this paper. The operations of the mission will specifically be in New Jersey in connection to other units designated in other countries on the same network. The unit will be set in the existing health center at the selected location in order to improve the services as demand rises, which are unavailable in the country. The new health unit is also set to act as a center for the campaign and awareness for advocating better health services and advices to the increasing cases of youth pregnancies. Justification /Rationale Teenage pregnancy is a real threat to the society in terms of the social and economic development status quo of the people and their existence. The problem has caused many cases of maternal death due to lack of better health cares on such cases. However, the problem occurs for lack of awareness and advices to the young relationships such as sexual education in order to avoid the unplanned pregnancies. This results many families from riches to rags and eventually begging since the parents of the children were not prepared for their task ahead. The real solution of spread of Teenage pregnancies lies within each one in the country to participate in its eradications, which will be through education of sexual consequences. It’s very much sure that women are infected at a younger stage than men. In this case, the teen pregnancy has become a priority of the New Jersey state. This is because of its complexity as their expose themselves in the poor social and economic conditions. This status includes low attainment of education, overcrowded or inadequate housing, language barriers, literacy, and violence in the families, negligence, and gang abuse. The teen pregnancy also causes their involvement in crime or in gangs as part of its risks. This is because they have limited access to opportunities and resources and instead involve themselves in such risks as an option for their livelihood. Different researches conducted have shown that, many adolescents people who have low expectations in their achievements in education and who fails in academics are more likely to involve themselves in sexual activities at earlier stages and become pregnant. Lack of communication skills and support from other people also are associated with the sex at early stages. However, bearing a child at a tender age can affect to long-term effects on all parties involved such as the teen mother, father, and the infants. The effects include higher risks to the infant of being born prematurely or with low birth weight. They are also likely to be born into families with limited economic resources and education. Problem Description Since 1992, there has been the impressive gains made in reducing child bearing and teen pregnancy and childbearing but the gradual decline in births to adolescents appear to be leveling-off in New Jersey. According to the Guttmacher, there are 23,080 teenage pregnancies of less then 19 years old, which occur each year in New Jersey. However, the majority of teens’ birth (77 percent) is unintended or unplanned. New Jerseys is the second wealthiest state in the nation, and should be ranked at the bottom on the list of the state in prevalence of teen births and teen pregnancy. However, New Jersey is ranked significantly higher (20th) than the other states for the rate of teen pregnancy (Denk 2008). The Social and demographic factors vary greatly across NEW JERSEY municipalities. Youth living in low social and economic circumstances are at high risk for teen pregnancy and STDs/STIs. The data by Kids Count illustrates that teens living in the top 20 largest New Jersey cities account for 45 percent of all state births to teens ages 10-14, and 55 percent of births to youth ages 15-19 ( NJ Kids, 2006). Kids Count Data also show that compared to statewide data, the top 20 largest New Jersey cities have: a greater proportion of black and Hispanic residents of 72 percent vs. 26 percent statewide. Higher poverty rates of median income of $31,688 vs. $65,282 statewide and 27 percent of children under age 19 living in poverty, higher unemployment rates 14.8 percent vs. 10.2 percent statewide. There are also more households headed by a single parent of 47 percent vs. 22 percent statewide; lower academic achievement of less than 50 percent of youth passed 8th grade language arts, math, and science vs. statewide rates of 72 percent, 62 percent, and 77 percent respectively. There were also lower graduation rates of 2004/2005 school year rates of 66 percent vs. statewide graduation rates of 84 percent. Goals, Objectives, Expected Outputs /Inputs and Indicators Goals The project has the overall goal of serving the teenage members of the society to be aware of the risks of early and unplanned pregnancies and treat them in an effort to decrease different cases of maternal challenges. The unit project also aims at improving the maternal services in the community to the existing health centers. Objectives >counseling and testing the teens on pregnancies using innovative approaches such as door to door testing >Care of the pregnant and prevent new cases of teenage pregnancies. >changing the community’s approach towards teenage pregnancies in order not to discriminate the victimized youths >Reduce new teenage relationships which results to the early pregnancies >Provide referral services in order to avoid more deaths on the maternal challenges. Expected outputs The following are the expected outputs Increase the number of people accessing the health care services by addressing the importance of being given the maternal education and care. Increased in number of pregnant teens to access health services care and treatment Advice the youthful couples on what to do and what to expect in the period Educate the large population to uptake the pregnant teens person in order to live in harmony Indications The following will show the indication of the out comes: >The number of people accessing health care services should not be discriminated due to their status, age, or gender. >The number of referrals received or reported to or from health facilities and to postnatal care support services. Methodology Methods are the activities, programs, services, or interventions that will take place to achieve the objective. Different methods will be incorporate including purchasing the high-tech machine to be used in the maternal laboratory, train the staff to have quality knowledge and skills for advising the youths on sexual matters. These methods shall incorporate evidence-based or promising practices and be appropriate to the youth’s age, gender, culture, and sexual experience. The unit shall also have a section where the youth will be explained on different types of relationships including the gays, lesbians, and bisexual and offer them advice on how to live without discrimination. Institution Capacity The medical unit is operated depending on how the managerial levels are arranged. The Medical Director is the head of all the operations in medication part where as Nutritionist leads in the side of dietary. Other staff includes the social workers and an accountant. A clerical officer is also on board in order to lay out other administrative tasks such as documentation and record keepings. A doctor will be needed every month in order to review the medical records and analyze the operations of the facility. Sustainability This will be achieved through: 1. Our organization has action plains of income generating activities i.e organizing fund raising with various donors and well-wishers who can support to uplift the community and the facility itself. The facility will welcome the volunteers in order to save the costs of casual labor and have efficient work to other staff as they get the support to serve the clients. 2. Among the fund raised, the project will involve itself into Saccos, to enable members borrow and repay with a very low interest that will enhance the up keeping of the project. The amounts paid as interest will be used to run the daily errands in the project. 3. The use of participatory approach in program planning and implementation as well as home based approach in the care of those affected, capacity building of peer and in collaboration with other implementing agencies and government department, economic empowerment of the community and fund raising from project relevant donors. Collaboration and Community Support The unit will collaborate with the community leaders in order to access the interior locations to offer the services. This helps the social workers to create awareness and campaigns in order to prevent the prevalence of pregnancy and reproductive services. Budget Narrative (Justification) The project is planned to cost a maximum of $300,000 in order to execute the structured serves for the society. It is aimed to serve more that two municipalities as Paterson health centre serves many people from different services due to its central point. However, the HSG is offering grants to different health care services and the project is qualified to get the $250,000, which is given to the centers, as it serves more than two municipalities. The award of HSG funds is contingent upon the continued receipt of these funds from ACYF. There accurate information of the budgeted makes it to be rated highly on other projects and have a high chance of being rewarded. The funds granted from different financial well wishers and granting organizations will be utilized in paying personnel including the salaries and fridge benefits, or he administrative and supervision. Others will be spent in consultation including the pro-rated audit services; others will be utilized in office expenses such as office supplies, telephone printing, postage, computer and other related data processing supplies. The expenses of the program including the meetings, supplies, campaign materials; costs related to conducting and collecting the research data in an effort of creating awareness. Some funds will be spent in training and continuing different promotional programs or meetings for the professional development. Other expenses include travel i.e. work-related mileage reimbursement; equipments including the computer lap tops and printers for the administrator. The facility costs also are budgeted incase the existing health centers require any costs for the sage of their facilities including the furniture, beds and such. Project Evaluation The facility’s operations will be evaluated every month where the doctor will analyze the medical history which have happened and define the challenges occurred where as the accountant will brief the Medical Director on the trend of the financial gains or losses encountered. This will arouse any interests to review or not the day-to-day operations. The dietary section will also provide their budget every month in order to determine the costs of the facility in terms of purchasing the products required. The profitability of the facility will determine the amount to be spent in the nutrition. Different challenges have been identified in order to promote the operations of the medical unit. This is includes the sound theoretical framework of the health beliefs in the society. Such beliefs include change of the teenage behaviors by advocating pregnancy analogy with different program development strategies such as advertising and other sources of expertise. The facility also will incorporate the required cultural signatories of the youth depending with the age, gender, and sexual experience that will improve the youths’ moral standards. It will ensure that it utilizes the sound theoretical framework especially the motivational theory, theory of the planned behavior and the stages of change. The social workers will involve the accurate medical content, with grounded scientific facts of sex in moralistic tones in order for the youths to get the appropriate information concerning the issue. The programs involved, especially in advertising, campaigns and other different forms of creating awareness will ensure that they demonstrate the effectiveness on reducing the sexual activities in pregnancy, HIV/AIDS and also STIs/STDs or even the increased uses of contraceptive drugs. The unit will also administer the engagement of youths in development activities that advocates the decrease of sexual risks behaviors, which are known to cause the risky outcomes in order to assure a likelihood of success in life.
