Lock Out-Tag Out, LOTO is a safety measure for ensuring that no one is exposed to the danger of energized parts during the periodical maintenance and servicing of equipment. There are instances where the standards of LOTO are not applicable thus calling for exceptions. The two exceptions are:

 When exposure to unsafe energy is curbed entirely by unplugging the equipment a power outlet and where the person carrying out the maintenance and servicing has absolute control of the plug. This is applicable only in cases where electricity is the sole source of hazardous energy to which workers may be exposed. This exception includes several portable hand power tools and other plug and code connected equipment and machinery.

 The second exception of LOTO procedures covers the procedures performed on negligible tool and equipment adjustment. This involves the cases where the worker is performing minor tool replacements or negligible servicing operations that are repetitive, routine, and directly connected to production occurring only during the process. Despite this exception, the employees carrying out the maintenance and servicing must prove that they have sufficient and effective alternate protection to guard against the potential risk of human interaction with energized machine parts (Williams, Williams, & Geller, 17).

In order to develop and document appropriate LOTO procedures, I will follow the following steps.

Find out all the machine that requires maintenance and servicing
Identify all the types of equipment, which may be energized, or release stored energy unexpectedly during its maintenance and servicing.
Determine the steps necessary to carry out maintenance and servicing task
Review each of the determined steps in line with the potential of causing danger in all energy forms.
The questions, which must be, answered during the annual evaluation for overall effectiveness covers four cardinal elements of the program, general, procedures, device removal, and training.
