Diabetes is a very challenging and difficult disease. Nurses are required to actively use their knowledge and experience to ensure the physical and emotional wellbeing of diabetes patients. The disease is mainly noted to have a wide range of metabolic abnormalities (Akohoue, Patel, Adkerson & Rothman, 2015). Although the medical management of the disease has improved over the years, significant challenges are still being experienced in different care settings. Several core measures are used to enhance the quality of the care provided to diabetes patients.

Diabetes HbA1c Measure

The Diabetes HbA1c measure is used in care facilities to measure the percentage of patients with type one or type two diabetes, whereby the most recent HbA1c which is greater than nine per cent (Shaw, Killeen, Sullivan & Bowman, 2011). It considers patients aged between 18 and 75 years. The identification of patients with an HbA1c value greater than 9% allows the caregivers and the nurse to focus on those with the highest risk of suffering from the adverse symptoms of the disease. While implementing the core measure, there are inclusion and exclusion criteria required to allow all the caregivers to record and report the data in a similar manner.

Role of BSN Nurses

BSN nurse plays an indispensable role in ensuring the use of HbA1c measure is effective and successful. They are required to be actively involved in the recording and reporting the data required as per the set-out inclusion and exclusion criteria. Moreover, they are tasked with the duty of ensuring the continuity of the process. Finally, BSN nurse help in identifying the risk factors so that those at the highest risk are focused on.

Acute and Chronic Health Problems

Nurses have the duty of ensuring that the patients receive the best care for the chronic conditions that they are suffering from. To achieve this, it is imperative that they identify the various risks factors which may in one way or the other affect the physical and emotional wellbeing of the patients. Moreover, nurses need to be responsive to the needs of the patients. By identifying these needs, it is possible to come up with the best intervention which will help counter the health problem (Wagner, Davis & Schaefer, 2012).

Difference between COPD and Asthma

One main difference between the two conditions is that asthma affects smokers and nonsmokers of all ages. OCPD on the other hand tended to be more common among the elderly with smoking increasing one’s chances of having the disease. Secondly, COPD causes inflammation and damage to the airways and the lung tissues. Asthma on the other hand results in inflammation in the lung’s airways alone.

Misdiagnosis and the Role of nurses in Helping Patients

When acute or chronic diseases are misdiagnosed, the interventions which will be designed will not be effective in ensuring the physical and emotional wellbeing of the patient. Clinicians are there required to correctly match the symptoms with the diagnosis so that the right intervention is designed. Shaw, Killeen, Sullivan & Bowman (2011) adds that nurses must always ensure that they identify and respond to the needs of the patient with chronic diseases by addressing the various risk factors which impact on their health.
