Health assessment is key to a nursing career. One can build confidence in understanding and respond to a patient’s circumstance by practicing and developing the knowledge and the skills of physical and health assessment (Jarvis, C. 2013). One has to listen carefully to hints and cues from their patient. It is the signals from patients that can guide and control one’s questioning and physical examination (Jarvis, C. 2012). Carrying out the health assessment and physical examination project in a group of four nurses can extend one’s inter-personal skills and boost their knowledge further.           
 
The group leader creates a common purpose. They present communication and give vision to the group. They clarify objectives, ensuring that each team member is participating, committed, and gets motivation. They ensure members make decisions well. The best mode of making decisions in the group would be by way of voting. Members should vote to pass or reject an idea.           

Another member of the group could take on the role of the thinker of the group. The thinker will collect and analyze data. They will listen to what other members; the patient and all the interviewees say and watch what happens. The thinker should be the one to provide solutions quickly after anticipating problems. 

An achiever in the team will strive for results. They ensure there are no delays and challenge assumptions, presenting improvements.         
The group should analyze and understand the task. They must know the time frame for the interviews and the process of interviewing. The task, in this case, is interviewing health care discipline practitioners and social workers about the comorbidities disease of the old Chinese female called Mrs. Tien.

Once the group has evaluated the task set, make sure all have assignments to tackle and that you all reach a consensus on what needs to be done. Make a schedule for the work to done to see areas that need contributions from members. The plan could include:

  • Title for the project: Comorbidities Disease
  • List of team members
  • A clear statement of what the goal is the tasks and questions to be included in the interview. Assignment of issues to be asked, to each member.
  • Deadline for handing in:
  • The presentation format.            

Ensure there are procedures for communicating, monitoring the work being done, solving challenges that arise with tasks or interviewees.  Appraise the timing of the work; ensure the group is working on the set plan. Get everyone's email address and identify suitable meeting times and places.                                                                                                   

Ground rules are important if the group intends to have an active working group (Oakley, B. et al. 2004). Ground rules will be the background for a strong working relationship based on an awareness of the expectations of each other. Group members may feel discontent as they adjust to how the team works.  There may be a difference between each other’s expectations and reality. The group might start experiencing arguments at this stage and sub-groups may start forming.  However, such a stage of conflict needs to be carefully controlled.    
   
Resolution mechanisms are important in the group (Bindley, J. et al. 2009). Members then begin to settle down as the team resolves some of its differences and makes adjustments.  Each member must feel valued and their views recognized. Respect, harmony, and trust must be developed, giving all members a sense of self-esteem. Team members gain confidence, perform well jointly, and can openly discuss what they think. They feel proud of being a member of the group. The tasks and objectives set are then achieved by the joint effort.
