NURSING INTERVENTION NURSING INTERVENTION The abnormal treadmill test that has been indicated in the patient gives indication of possible complications that can develop to worsen the patient’s cardiovascular disease when the right interventions are not taken very immediately (Ralph & Taylor, 2005). In the light of this, five major nursing interventions are recommended to control the risk factors of cigarette smoking and high fat diet, which are obvious indications to why the abnormal treadmill test exists. The first intervention will be to advise the patient to quit smoking. It has been said that 4 out of 5 people who smoke are ready to quit (Landy and Janes, 2001). This means that there is a moral will for most smokers to quit their practice. The challenge with these people has however centred on the motivation to do so. Motivation to take a step of this nature however comes in many different forms, including a conscious realisation created for the smoker on the need to quit smoking. As a nurse, giving these people the needed morale to quit is important and can start with a simple advice on the need for them to quit. The chances that the patient has received similar advice are higher. But in most of the cases, smokers are faced with the dilemma of authenticating the advice they receive (Proctor, 2000). This is because they tend to see people who advise them as merely disagreeing with them on the practice of smoking. Once a professional like a nurse is the one giving the advice, it will be seen as a health intervention needed by the patient if he wants to live a healthy life. The second intervention will be the use of nicotine replacement therapy, with particular use of nicotine gum. It has been noted that the problem with most smokers who are addicted is that they are not addicted to the cigarette itself but to nicotine (Proctor, 2000). In effect, an intervention that takes away their addiction to nicotine can easily control the smoking of cigarette. It is for this reason that nicotine replacement therapy is recommended as one of the nursing interventions that will be taken for the patient to ensure that he controls the risk factor of smoking. This is recommended, considering that the 54 year old patient might have started smoking very early and would thus need a more pragmatic intervention that builds on the advice that is offered. The selection of nicotine replacement therapy is however an intervention that will be executed in consultation with other health experts, particularly dentist and doctors. This is because there are cases where nicotine gum has been noted not to be good for some patients (Proctor, 2000). In such a situation, nicotine patch will be most recommended for the customer. At the end of the day, it expected that the nicotine replacement will help in reducing withdrawal symptoms in the patient. The third intervention will be a follow up on quitting where the nurse will show maximum concern to the patient for the situation he finds himself in. When used as a nursing intervention, follow-up can be referred to the process of evaluating and action that was previously began by visiting the grounds (Iowa Intervention Project, 2006). This is done in a much formal manner where the nurse goes with specific data that needs to be gathered from the ground. In the current instance, the follow up will be conducted on the patient with a documented checklist on progress with the cessation of smoking. By following up, the nurse will be better placed to evaluate success progress. Based on data that is gathered on the success progress, the nurse will be better advised on what the way forward should be for the patient. Under the current circumstance, there are three major types of decisions that may be taken as a result of the follow-up. The first is the need to continue the ongoing interventions as they are. The second will be to modify the ongoing interventions either upwards or downwards. The third will be to change the ongoing intervention all together. The fourth intervention will focus on the risk with fat diet, where there will be a calorie reduction meal outline prepared for the patient. Most patients do not have any professional knowhow about the meals they take in and the impacts these create for them. For most of them, the need to satisfy their hunger with a piece of meal is all they need. This is however a wrong dieting approach. Given the current circumstance where the patient has been noted to have high fat levels, it is important that the daily calorie consumption level of the patient be controlled. This will be done by preparing a professional dietary chart that will determine the quantities of food that will be consumed by the patient and in at what times. This will be an adherence dietary chart. Professionally, it has been advised that the risk of eating at the wrong time may be as risky to a patient as the risk of eating the wrong diet. Because of this, the dietary chart will focus on both the food to take in and the time to take in. Finally, a physical exercise schedule shall be prepared for the customer so that existing fat in the body can be burned down. To be efficient with controlling the risk factor with high fat diet, it is important to tackle the existing impact of the fat diet that has already been created. After this has been done, chances that any other preventive intervention can have immediate results are higher. It is because of this that a regular body exercise schedule will be designed for the patient. In line with the patient’s poor treadmill test, it will be important to focus on endurance test for the customer (Dick, 2004). This will be necessary also in improving the patient’s chances with improving with treadmill test (Dick, 2004). Through various endurance tests, the patient will be able to burn fats very quickly, whiles developing very healthy cardiovascular endurance system that will improve results with treadmill test.
