Nurses have gone beyond their role as caregivers to become coordinators and key integrators of care to improve efficiency in the care delivery system. The aim of the Innovative, Nursing Care Delivery Model is to bring attention to improving the health care delivery systems. It also promotes, replications of quality care from some of the models that emphasize interdisciplinary collaboration, in the care delivery systems, and offer good organization in the workforce in the rapidly changing health care systems. Health care systems emphasizing the importance of innovation include the team nursing and primary care model. Although the models differ, they have common themes emphasizing the importance of innovation in care delivery (Stull & Lantz, 2005).
Common principles in the nursing care delivery model
The innovative delivery care model calls for a team approach to provide interdisciplinary care to patients. This is essential in the provision of high-quality care to the patient for quick recovery and reduces hospitalization costs. The model calls for continuity of care with the patient even outside the hospital setting through follow-up (Venturato & Drew, 2010). The role of the nurses has been elevated to care integrators, as opposed to the general role of the nurse as a care provider. This is important in ensuring the integration of care, as well as continuity of care delivery system. The innovative models push for the home as the setting of primary care. It relies on the patient’s home settings as the center of care in understanding the patient better. The care models focus their targets on older adults who are major primary seekers of healthcare. Patient focus is a major principle of the care models. Their major focus on is the family and are patient-centered to achieve greater responsiveness to the patients’ needs and wants. Innovation care delivery models try to solve specific problems thus are goal-centered. They put emphasis on quality care, provider satisfaction, and reduce unsustainable costs and utilization (Niederhauser et al., 2012).
Conclusion
The innovative model emphasizes a team-oriented primary nursing model to provide care in inpatient and outpatient settings. Nurse practitioners and clinical nurse specialists have a chance to provide significant-quality care and leadership in the health care system. The innovative care delivery models are essential to ensure the quality of care, reduced cost in health care as well as enable good organization and achievement of health care goals and objectives. Patient and family engagement in care is important in ensuring continuity of care to the patient. If health care systems can make good utilization of innovation in health care delivery systems, the quality of care will also improve significantly (Stull & Lantz, 2005).
