Most of these models target people who are heavy users of health care mainly the elderly. The model also shows a shift towards the offering of patient-centered services. The patients and their families are actively involved in the delivery and planning of health care. The patient’s needs are given priority.

The use of modern technology in health care is another key characteristic of innovative models. New technology has been incorporated into the model as a way of improving efficiency. Although it is not possible to have a complete overhaul of the traditional care model, nurses and other health care providers are advised to adopt practices that improve the quality and cost of health care.
