"Nursing Management and Patient Care" is a great example of a paper on care. Within the community, the majority of people suffer from four main diseases like diabetes, hypertension, alcoholism, and obesity. These are lifestyle diseases that are considerably impacted by the external environment and social determinants of health can address these diseases (Allener, Rector & Warner, 2009). In the current environment of increasing healthcare costs, a holistic approach to health becomes a critical issue within the community healthcare delivery. As such, nursing leadership and advocacy have emerged as an important paradigm that can be exploited for improving and improvising the health of the community. Nursing advocacy promotes the empowerment of the people with regard to diseases and helps them to adopt change in lifestyle. Most importantly, Nurse leadership becomes a pertinent issue within community healthcare as it tries to identify and evaluate the social factors that are responsible for a healthy society.

The community has a large number of immigrants from Mexico who do not speak English. They are poor with no social security as most of them are illegal immigrants. Thus lack of financial resources, education, cultural dynamics, and other social issues like regular employment, hygiene, etc. become important factors for diseases like diabetes, hypertension, alcoholism, and obesity. Indeed, illnesses are mainly caused by improper diet, unhygienic conditions, lack of education, and lack of awareness regarding healthcare issues and a healthy lifestyle. As a nurse leader, I would like to focus on the empowerment of the community regarding healthcare issues. The grant proposal would address this area. This is one of the most important modifiable risk factors that promotes a better understanding of the causes of diseases and helps people to change their lifestyle for improved health outcomes.

A transformational model of nursing leadership would be most appropriate for empowering the community with information regarding their health. It promotes a participatory approach that brings about behavioral transformation in the people. Scholars assert that nurse leaders are endowed with transformation leadership that involves other healthcare workers in the decision-making process, especially the patients or community in general (Motacki & Burke, 2010: McGuire and Kennerly, 2006). This is a hugely critical factor because each community has different needs and requirements depending upon its social environment, income range, employment, amenities available, etc. The participation of the people is very important to define the needs of the community. Consequently, the empowerment of the community significantly becomes the catalyst for the change.

Nurse leadership become agents of change and help build constructive relationships based on empowerment. It encourages a holistic approach to solving all the problems and makes efforts to incorporate mutual interests rather than individual ones. Empowerment leads to a better understanding of not only the diseases but also the socio-environment issues like poverty, education, employment, hygiene becomes important for nurses to evolve innovative ways to address these issues and promote better health. Thus the community becomes aware of the importance of proper diet, hygiene, stress management, etc. These are important issues that affect health. The community is empowered with requisite information so that it can make effective decisions with regard to health for higher health outcomes (Ryles, 1999). Thus, the empowerment of the community would greatly facilitate improved health outcomes of the clinic population.
