Case management: Dementia in the elderly Introduction This implementation plan aims to present what needs to be put into consideration when taking the necessary measures in caring the elderly people in the society who are vulnerable to dementia. In addition, the team major task will be performing surveys before the implementation. The team members will conduct a research to determine the nature of this condition. This is crucial since it will give an estimate of the amount of work required to be done and the amount of resources that are required in the overall plan.
