Summary of Unit 2 Discussion Affiliation Introduction The scintillating discussion amongst the members and the Dr. Griffin centered on the comparison of factors that shaping external environment of long term care facilities. The task was to identify a nursing facility within the students’ locality, thereafter a brief discussion, applying the Four Levels of Environmental Proximity Model, and identifying an example to support each of the four proximity levels. This paper summarizes the 16 primal discussion points from the class members and the contribution contained within the focal postings.

Fellow classmates have provided a raft of examples and the corresponding lists of examples supporting each of the four proximity levels. Facilities and Environmental Proximity ModelBishop Pearce provided the example of the Waterbrooke Assisted facility, located in Elizabeth City, NC. The facility provides personalized care including three meals and snacks a day, physician and psychiatric services. On environmental proximity, he identified each of the four levels drawing the work by (Sigh, 2010) to be the foundation on which he drew the four levels on proximity.

The work drew a number of replies, most notably Kathleen, who wanted to know if the facility were next to hospital, would it have been a source of hindrance or help. Singh, D.A. (2010). Jones and Bartlett PublishersYvette De Leon, on the other hand cited the Arbors of Port Warwick Facility that is in Newport News, VA. The hospital provides 24 hours services to the residents, though only a meal day is provided. Just like the first contributor, Yvette cited (Singh, 2010), when updating the four Environmental Proximity Model.

Kathleen Almeda, on the other hand, gave Dominion Village at Williamsburg that is owned by Five Star Senior Living.  Dominion Village offers assisted living, memory care, outpatient rehab services, and respite/short-term stays. Felisa Artis provided the example of the Consulate Health Care as the facility of choice, which specializes in post-acute care, operating 200 centers in 21 states. However, her focus is the one that is at Norfolk, VA. She explicitly provides the examples to match the four levels of proximity and a discussion ensues between the classmates and her.

Jennifer Supples provides an example of a facility located 15 minutes from his place of residence, Willows at Meadow Branch which is part of the Consulate facilities. However, he does provide examples on the environmental proximity models. The level of services provided in the facilities were also different, for example, Paola Williams mentioned that Hopkins center for Rehabilitation and Healthcare, Brooklyn NY, provided a maximum physical, social and cognitive function. Another classmate’s (Yvette De Leon) choice facility merely provided basic care, yet the service was available 24 hours daily.

Second, examples supporting the each of the four proximity levels were different from one posts to the next, for instance Ashley’s choice facility Country Village Retirement Community level one proximity example is the extensive rehabilitation services. Second proximity example noted down by Ashley is for the facility to build a good solid relationship with hospitals within the locality and finally a level 3 proximity example would be the turnover rate of CAN’s in nursing facilities. Courtney and Bishop responded to the post each commending and seeking answers, for instance Bishop requested to know if there were any areas which could be improved within Ashley’s choice facility.

ConclusionSince this two page paper cannot succinctly summarize all the postings from the discussions, the text above merely expresses the spirit of the discussion. Nonetheless, the point that is not worthy of mentioning is that all the facilities mentioned in the discussions were different one from the otherReferenceSingh, P. D.
