Assessment Assessment After conducting a physical assessment on various parts of one of my male patient’s body, I managed to figure out a number of normal and abnormal findings. Some of the normal findings I found on the patient’s head include presence of all the fourteen facial bones. These particular bones are the ones that join the sutures. The patient had a general size of the head. Another significant normal finding I managed to figure out is that there were no deformities on the head. I also managed to figure out various abnormal findings that include lesions of the skin. There were scaliness and various lumps on the scalp of the patient (Bickley & Szilagyi, 2008).
I also performed physical assessment on the patient’s eyes and managed to figure out several normal findings. One of the findings is that the patient had normal vision ability on both eyes. The other finding was that there were no irritations or infections on the eyes. Moreover, the movement and the condition of the pupils were normal. The lining of the inner surface of the patient’s eye depicted to be normal. The pupils were round and had black color as recommended on normal eyes. The abnormal findings I managed to figure out include dryness in the eyes. The eyes also depicted to be red, which is an abnormal issue. The other abnormal finding was that there were changes in visual acuity on the patient’s eyes (Bickley & Szilagyi, 2008).
My other obligation was to conduct the physical assessment on the patient’s ears. The normal findings I managed to figure out include presence of the three compartments that make up this particular organ. No deformities I managed to find on the patient’s ear. The other normal finding is that the patient could hear and understand speeches that I made. I managed to conduct this particular assessment while making sure that the patient was not looking at the movement of my lips. The abnormal findings I found on the patients ear include presence of a discharge. There were skin lesions on the ear and the patient complained of feeling pain after making movement to the auricles (Bickley & Szilagyi, 2008).
The normal findings that were presence on the patient’s neck included lack of lumps. Moreover, there were no swellings available on the patient’s neck. After moving the patient’s neck, I managed to find out that the neck not limited to active range of motion. The lymph nodes on the patient’s neck were movable and discrete. After making a thorough assessment on the neck, I found out that the alignment of the trachea with the midline was normal. Some of the abnormal findings that I managed to figure out include the presence of scars on the neck. There were also skin lesions on the patient’s neck (Bickley & Szilagyi, 2008).
I was also supposed to conduct a physical assessment of the patient’s nose. The normal findings I managed to figure out include lack of any discharge. The external part of the nose was not deformed. I also managed to figure out that there were no deviations on the septum. Some of the abnormal findings I managed to figure out include presence of unusual skin markings. The patient also explained to have unusual change in smell. Moreover, the patient explained that frequent nosebleeds occurred sometimes (Bickley & Szilagyi, 2008).
Some of the techniques that I employed while conducting the physical assessment include palpation. This is using fingers when feeling various part of the patient’s body (Lippincott & Wilkins, 2008). The other technique that I employed was percussion (Lippincott & Wilkins, 2008). While using this technique, I could softly tap the patient’s body still using fingers. I also managed to use another technique known as auscultation (Lippincott & Wilkins, 2008). While using this particular technique, I managed to take considerable time to listen to functions of various body parts of the patient.
Some of the instruments that I used during the physical assessment include pnematic otoscopy. This instrument is eligible to use when examining the patient’s ears (Lindh, 2010). The other instrument that I used is known as endoscope. This particular instrument was necessary for examining the nasal cavity (Lindh, 2010). While examining the interior part of the patient’s nose, I managed to use nasal speculum. I used another instrument known as typanometry. This instrument was of significant help for detecting sound transmission (Lindh, 2010).
During the physical assessment, I managed to ask the patient several questions. Some of the questions included whether he had been under any medication before. The other question was whether he had been using any drug or substance. I also managed to ask the patient about his family history. Whether there were some of his family members who had been diagnosed with any disease (Viljoem & Sibiya, 2009). While having the conversation with the patient, I considered several cultural awareness issues. Some of the issues I considered include whether the patient was feeling comfortable when discussing various issues related to his lifestyle. I could also consider his age and race when administering him medication. I also considered some of the specific diseases that occur in regions where the patient came from (Talley & OConnor, 2009).
