The sustainability of human beings depends upon the physical condition as well as on several other factors. The basic structure of health care system deals with individuals, groups who thrive to dedicatedly work for the promotion of overall health of people all around. The general structure of health system in a society comprises of private health service providers, occupational health and safety legislation, health insurance organizations, and presence of any unit working for the education and promotion of health. In spite of the presence of the World health Organization and other international health regulating body the overall standard of health of the majority of world population has not taken any drastic improvement. Development of goals for health in the world’s poorest countries still lacks planning and the spread of chronic diseases and epidemics has not been encountered properly. Inadequate health policies still persist in many nations of the world and the overall health standard is falling drastically. On this context it is quite important to know the international health care system. The older citizens are quite susceptible to various ailments, and dementia being one of them. The discrimination faced by the older people relating to their health associated problem needs to be studied carefully. The paper focuses on the study of the article” Sustaining citizenship: people with dementia and the phenomenon of social death”. The paper deals with the nature of relief provided to the patients of dementia and the concept of social death. Problem: What role does the society play in providing relief to the sufferers of dementia and sustaining their citizenship? Purpose of Study The global health care and the international health care system require close study owing to the importance of the matter of its concern. Health related issues have gained much importance in the modern society though there is still lacking of appropriate measures to be incorporated. Certain target groups has been vulnerable to the lack of proper healthcare facilities. It is important to study the relevance of the international healthcare system and their efforts aimed to provide suitable solutions. The problem of dementia has been encountered by people all around and often they are considered to be socially dead individuals. The social disregard faced by the older citizens is studied. The benefits and the nature of help they receive have been closely investigated. The discrimination faced by the diseased people owing to their inactive participation as citizen has been closely highlighted. Literature Summary: The numbers of people suffering from dementia is also considerably on a rise and the three tier health system is working for the prevention and treatment of such diseases. It has also been noted that the cost of healthcare for dementia patients increases significantly with the change in functional impairment. There is a growing awareness among the people to prevent the occurrence of such catastrophic disease which is hampering the health of so many elderly people worldwide. People affected with fatal illness for a long period of time and those found to have a tendency of losing their personality are often declared to be socially dead. The viewpoint of the careers of people with dementia is detrimental in providing a social status to the patients. Four sets of beliefs were considered. A portion of the people in charge of the patients held the life of the sufferer’s as of no value and they wait for the patients to die. A second set of people held the proposition that the life of the patient is less of worth and they also eagerly wait for the patient’s death. A third group of carrier viewed the life of the patient as less worthwhile but was never looking for the death. Whereas the final group believed belief that there is still a lot of worth and meaning behind the life of the sufferer. The attitudes of the person in responsibility of the diseased were also differentiated based on their response to the diseased. Some of them ignored the patients and provided little or no care to the patients. There were some in charge of the patients who provided a little amount of attention towards the sufferer. While some of them provided relief as and only when required, and a significant portion of them provided proper relief based on the demands of the patients. The discrimination in the behavior caused adverse effects on the patients and some of them developed massive complications and their health condition faced rapid degradation. Research Methodology The research methodology adopted for the particular topic is based on the primary and the secondary method of study, which includes both qualitative and quantitative methods of research. The quantitative method of data involved the study of the global policies of the heath issues and an idea was tried to generate from them. Whereas the study of the health factors on the people affected with dementia and their implication on a particular section of the society did took the help of numbers and figures and followed the quantitative approach. The data collection and the source used for the particular topic were based on the secondary resources and the help of journal was taken. The research methodology involved the observation of 15 practitioners, 8 community nurses and 7 seven workers and also involved 50 patients suffering from dementia. The mode of data collection was not interview rather written data was considered among the sample of participants. (Brannelly, 2011, p.665) Findings: The problem of dementia encountered by the people has been an ever rising one. The facilitation of the proper care and services provided to the people from this particular range of society by the social workers and the doctors and nurses reflected a varied approach. While it was found that a significant portion of the people were forced to be under the proper treatment of the dementia. 10 of the patients were moved into the nursing care .The treatment and care received by the patients was based on their expectations and their preference. Planning of services was done in accordance with the patients view point and the understanding o the people affected with dementia was taken into account. This provided social regard to the person and provided mental satisfaction during the treatment process also. 39 out of the 50 patients considered were found to be provided with social regard (Brannelly, 2011, p. 667), However instances of social disregard among the people affected with dementia are also widely evident. Often it was cited that home care was denied to the persons as the person in responsibility of providing care to the patient was not provided with any sort of health from the social workers and the patient was shifted away from his home to provide medication facilities. However it is to be agreed that there is also a huge burden on the people and the family of the dementia affected people relating to its care and service. The burden may also cause long term distress among the family members. So often disregard of the patients view is being done keeping these factors in sight. It has been found out that it depends on the practitioners to declare the people suffering from dementia as socially inactive or not. A majority of the practitioners provided good care to the people and made rational decisions regarding fundamental aspects of daily living. When the patients are unable to represent their views due to the criticalness of the disease, then the choices of providing care to them is conducted after studying the close history of the person and his choice and preference. Keeping in view of the seriousness of the disease, considerable development have been made in the diagnosis of the disease. Advancement has been made in the identification of genes causing the likelihood of Alzheimer’s disease and the prevention of dementia. The overall development in the health science has benefitted treatment for the disease of dementia. The nursing homes have added special dementia units as advancement to the care, which is providing considerable service to the people. In spite of the advancement several pitfalls were also noticed which included the increased cost behind the care. Patients seeking admission to the special care unit needed specific admission criteria and it also followed up with complex discharge criteria from the hospital. Appropriate use of psychotropic medication is in practice by the hospitals and the nursing homes for the treatment of the dementia. The proper care of dementia not only included proper medication facilities but also depended on proper individual domains and the societal policies overall Relevance to Nursing The impact of proper nursing facilities in the health service detrimental in the overall assessment of quality received by the people and more priority should be given in the matter. The patients should be treated with utmost care and the treatment should involve trust, generosity and empathy. The accessibility of health service is not done in a proper fashion due to diverse culture and huge number in population. Effective measures needs to be implanted to provide same service without any discrimination. The impact of proper nursing on these sections of the society is huge. It is to be remembered that the patients of dementia are old in age and hardly could look after themselves. Recommendations: There should be no discrimination by them while treating matter of dementia and other disease. The phenomenon for social death is relevant for understanding of the response to the older people suffering from dementia. The ideology behind social death is relevant for distinguishing their role play as proper citizens of the society. It is important not to treat the sufferers as socially dead but adequate service and facilities should be provided to them in curing of this huge proportion of the society. The people in charge of them should treat them in humanitarian grounds. The view point of the pessimist care takers needs to be changed in relation to their service being provided.
