Health is the state of being free from any injury or illness. Therefore population health can be defined as the concept where the state of a community is analyzed in terms of their disease (Fink, 2016).

Population Health is a broad area that covers the policies and research within the population. Each population has its advocacy on how health needs to be handled. The policies are influences by aspects such as the overall health outcomes. Each population health practitioner helps in administering health policies (Fink, 2016). A clinical nurse helps in the planning and coordinating of patient results, patients care and transition programs, quality improvement of the health care facility, and ensuring a smooth use of evidence-based practices (Marmot, 2005).

More to the clinical nurse, a population health center requires an executive Nurse educator/leader, he or she is in charge of the health center patient's overall care (Fink, 2016). More of her duties include organizational skills, critical thinking skills, and communication that pertains to the health Centre. The Executive Nurse plays a role in the management and administrative tasks such as hiring and budgeting. Social Determinants of Population health The primary goal of population health is to create an environment that thrives in good health.

The social determinants of the population include Health and Health care, Community and social Context, Educators, Neighborhood and built environment, and Economic Stability of the population.Various factors determine the economic stability of the population the employment rate of the community. You will find that a society with a higher employment rate has low mortality and knows different policies within the health Centre. Food security is key in health; if the population has a problem with the availability of food, then the likelihood of more health issues is high.

The basic needs of a human person need to be fulfilled in any population health Centre. More to food, there is shelter (Marmot, 2005).In populations that have problems such as flooding and famine, the health situation becomes an issue and required high intervention from the government. The illness cases become more, and the outbreak of significant illness becomes a problem. The overall poverty level of the population determines the impact of population health.With education, lies the pushing forces that health nurse practitioners can help overcome the population health crisis from early childhood education to tertiary education.

The population is educated in different aspects of health and how to mitigate the different crises. For example, amidst the Covid-19 pandemic, the population health practitioners are responsible for teaching their members how to manage the spread of the virus (Marmot, 2005). The education factor can be determined from the number of students that enroll in high school, several graduates, and the overall literacy level of the population.The Community and Social Cohesion of the population plays a role in the population health standard.

This determinant is affected by factors that include, Social cohesion, Incarceration, Discrimination, and the people's participation in health projects and programs.The neighborhood environment of the population can be explained by the environmental conditions, crime rates of the area, and access to good and healthy food (Marmot, 2005). In situations where the environment is harsh, for example, dry areas, the population health becomes a problem due to the poor living standards. The population health officers regularly need to formulate strategies that can help mitigate the social determinants of health.

The living standards of any population are the first tool that needs to be addressed. More to education, the population health officers need to use health assessments to be able to understand and propose any health policy that will help the improvement of the population (Fink, 2016). What is more, the members can apply strategies such as "Health to all" where every citizen has access to free quarterly checkups.Comparison of Public Health and Population HealthOverall we also need to have a comparison between public health and population health.

First, by definition population health is concerned with the improvement of health in the entire human population. In contrast, public health is a sector of health that deals with certain health matters, for example, hygiene or prevention of outbreaks (Fink, 2016). The Patient Protection and Affordable Care Act (ACA) popularly known as the Obama care, plays an impact on the overall population Health sector. The Act was enacted in the Year 2010 that offers new guidelines on the health offering and coverage in the United States (Fink, 2016).

The law can help in regulating the health providers, population, and insurance providers in the population health area. The secondary is the Accountable Care Organizations (ACOs) whose primary goal is to gather all health practitioners such as doctors, nurses, and care providers. Such laws and organizations, the population health are given the backbone to exercise their duties with the right policies (Fink, 2016). It is from such groups that population health initiatives can be enacted and be helpful to the immediate population.

ConclusionFrom the analysis above we can conclude and state that the social determinants of a population play a significant role in maintaining the overall health crisis. In community Health every person has a responsibility to maintain a good health condition. The laws and different organizations in the health care sector will help in the mitigation of further crises.
