Evaluation is a critical stage during the lifecycle of a project. The evaluation process is scheduled at the mid-term stage of the project in this case. It will also encompass monitoring or assessment. The process will be internal meaning that the project will not consult an external evaluator. The evaluation process will focus on a designated and specific process during the project’s lifecycle (Hicks, 2014a). In addition, the evaluation process will be formative by focusing on efficiency, effectiveness, and impact analysis.

 Effectiveness is the extent to which the project achieves or accomplishes its objectives. The process of evaluating effectiveness will also consider the cost of meeting these objectives. Other variables to consider include the actions and strategies used to meet the objectives of the project, their weaknesses, and strengths. The effectiveness of a project seeks to establish whether the objectives of the project are still valid. The determination of the project’s efficiency will assess whether the cost is proportional to the benefits. Cost in this case refers to human resources, financial resources materials used, energy, and time. The impact analysis will evaluate the impact of the project on the organization, stakeholders, and employees.

How the Evaluation Phase Will Make Substantial Contribution to the Project

The evaluation will make substantial contributions to the project by creating an analytical platform. It means that the evaluation process will establish whether the project is aligned to its objectives. In addition, it will establish whether the activities of the project have met its objectives at different stages (Hicks, 2014a). The evaluation will also provide opportunities for assessing the project’s resource allocation. Substantial contributions to the project will be made because the project manager will understand the outcomes, positive and negative outcomes of the project. The intention is to make improvements that will increase effectiveness and efficiency.

The evaluation provides crucial knowledge that can be used in the next steps of the project. Through the evaluation stage, the project has the opportunity to build knowledge and skills that support sustainable actions. In addition, communication with internal and external stakeholders will improve and methods of accountability develop.

How to Know If the Interventions Are Making a Difference to the Chosen Population

The chosen population is African American and Hispanic diabetic patients. Assessing the interventions and their impact on the population requires an assessment of the target population. The interventions will have made a difference to this population if there is a reduction in the morbidity and mortality associated with diabetes.

The target population should change their lifestyle and improve their healthcare (Hicks, 2014b). This means that the number of people within the population who are exercising should increase. This population should show a willingness to develop exercise plans and follow them. A preferable change is an increase in the number of people within the population who are following a recommended diabetic diet plan. The target population should show an increase in the consumption of foodstuffs such as wholegrain bread, brown rice, leafy vegetables, sweet potatoes, and peas (Brown, Mosley & Aldred, 2013).
