"Community Health Promotion Program" is a great example of a paper on the health system.
This project is aimed at outlining a community health promotion for individuals who live with diabetes in Calhoun County. The promotion is based on the precede-proceed model and is aimed at the prevention of diabetes through behavior and attitude change. One of the assumptions of the precede-proceed model is that health is that individual health, in a way contributes to the overall well-being of the community. To that effect, communities should always take up the role of ensuring that those who reside within them are of good health and a sound mind.

Under Social Diagnosis, which is the first phase of the model, assessment should be done within the community members that seek to identify some of the primary reasons for the occurrence of diabetes within Calhoun County. The second phase that deals with Epidemiological assessment should look into the determinants as well as the distribution of diabetes. This includes the fact that the genome organization of people from certain ethnic races is sensitive to diabetes, making them highly susceptible to diabetes (Paraska, 2012). The third phase, which focuses on the behavioral and environmental diagnosis, should seek to establish whether certain behaviors put an individual at a higher risk of suffering from diabetes. Similarly, it should determine if some practices could reduce the occurrence of diabetes. Environmentally, the assessment should determine if the risk of diabetes often increases based on the social and physical activities that a person takes part in. The findings should then be rated according to how significantly they affect the disease and how well something can be done to change the situation. In case this third phase is not planned properly, then the project is likely not to yield the desired results.

The fourth phase, which focuses on the educational and organizational diagnosis, should be done to reinforce some of the findings established in the early phases of the theory. After the factors that result in a high incidence of diabetes have been ranked in terms of significance and changeability, this phase should ensure that the organization of the health promotion puts more emphasis on the major causes of diabetes. It should outline how the identified factors can be averted (Paraska, 2012). The fifth phase on administration and policy diagnosis should look into some of the administrative issues that may hinder the enactment of the health promotion program. Similarly, this phase should put in place an ideal strategy that can be employed by the Calhoun community to reduce the incidence of diabetes.

The sixth phase, which deals with implementation should be aimed at converting the outlined strategies into action. The regulations and changes introduced by the Calhoun community in this phase should be aimed at ensuring that the incidence of diabetes reduces. For those already suffering from the diseases, the strategies outlined should ensure that they live much longer with the condition. The seventh, eighth, and ninth phases, which deal with evaluation should be put in place to evaluate the success of the promotion program. Process evaluation should assess how effectively the implementation process was carried out (Paraska, 2012). Impact evaluation should be carried out to determine some of the immediate outcomes. This should include the incidence with which patients with diabetes suffer from diabetes-related complications. On the other hand, outcome evaluation should be done to determine whether the incidence of diabetes has reduced in Calhoun County.
