"The Department of Health of the United States of America" is a great example of a paper on the health system. The U.S department of health's desire to identify the attitude of the sample group towards their respective workstations prompted a design of a questioner. The questioner has met the desired objectives by outlining lead questions that will assist the researchers while performing the survey.  The questioners are designed to ensure that the duration of a particular worker reflects his or her attitude. The researchers will be able to first identify the workstation of the worker and use the questions to narrow down to departmental details. The questioner has ensured that the researcher is able to identify specific details per department and make a decisive conclusion without generalizing their findings.

The questioner has also managed to integrate the essence of both open-ended and fixed-alternative questions. This will enable the researcher to use the answers given in the decision-making process. The survey has the essence of instructions to guide the target group in answering questions according to expectation. This ensures the data collected meets the expectation and at the same time avoid ambiguity while presenting the data (Thayer-Hart et al, 2010).  The intention of the survey is clear has details of the target group and the desired outcome is clearly stated at the beginning of the survey questioner. The idea of stating the aim of the questioner is a requirement since it will assist both the departmental heads and the works to be able to connect the question with their immediate environment.

The questioner has also avoided asking a personal question such as name and ethnicity. By doing so, the sample group may give answers without fear of intimidation. This will increase the accuracy of the data collected. The third-party influence will be minimized by choosing to avoid personal information while collecting data. The questioner has also avoided the double-barreled question effect, which ensures the answers being collected are based on specific aspects rather than a general approach answers (Thayer-Hart et al, 2010).

The undoing of the questioner falls on the arrangement of the question. The set time to collect the data is 15 to 20 minutes hence proper arrangement is needed to ensure the questions are answered within the set timeline. The final section of the question contains information that fits most in the first section of the questioner. This may prompt the researcher may be prompted to surpass the set timeline to collect and analyze the said data. The questioner also lacks the option where the sample group should decline to answer the questions hence giving the questioner a mandatory status. The data collected therefore may be biased since answers given may be unreliable due to the subjective nature of these questions.

While attempting to improve on the questioners, new questions should be added to improve the quality of the data to be collected. The questions will base majorly on the specifics of each department. Nature will determine whether the attitude per worker is either work-related or is based on other factors rather than work (Thayer-Hart et al, 2010). This will be done by focusing on general questions rather than personal information that may affect the reliability of data collected.
