"Eyes and Ears Plan of Care" is a well-written example of a paper on ophthalmology.
Conjunctivitis has a wide range of causes. It can be caused by bacteria or a virus. Certain times it does occur as a result of a certain disease or even allergy. These attack the conjunctiva leading to effects like the discharge of the eye as in the case of Marcus. Some of the major symptoms are Reddening of the eye, discharge from the eye, and itchiness of the eye. Diagnosis of bacterial conjunctivitis involves an examination of the symptoms by a health practitioner and also checking on the history of the patient. Generally, the diagnosis needs one to know about how to examine the eye, knowledge of differential diagnosis, being aware of strange conditions, and consideration of chlamydia. (Gibson & Benko, 2013)

  • Goals and outcome criteria

It would be necessary to know the various causes of conjunctiva such as bacteria, viral infections, and also certain diseases. It would be necessary to make the student aware of the symptoms associated with this disease so that anything that the student observes ceases to be strange (Yanoff M, 2011). In the case of Marcus, he should be told about the various symptoms such as eye discharge and also the reddening of the eye among other many symptoms.

Documentation of eye and ear findings: The documentation of the information follows a distinct order and format (Yanoff M, 2011). The first thing when documenting the findings is to ensure that an expected outcome is well defined to check if it follows the right standards. Providing definitions for basic terminologies used to source bacterial conjunctivitis the findings becomes the second thing. The policies are then authenticated.

  • Nursing orders

How to take care of ears: For people who work in areas where there is loud equipment, it is important that they put on hearing protection. Playing loud music in enclosed areas should also be avoided as it deteriorates hearing ability as time goes by and destroys the eardrum. Ears should also be cleaned regularly. Soft cotton buds that are moistened in warm water should be used. (Kongstvedt, 2013).

How to take care of eyes: Eyes should be shielded from excess ultraviolet light by putting on sunglasses. A regular visit to the optician for a checkup is important. Disorders or illnesses to the eyes can be detected early and the appropriate action taken in order to avoid deterioration (Kongstvedt, 2013). For the cleaning purpose of the eye, warm water on a moistened piece of soft cloth should be used to wipe the outside part of the eye (Sole, Klein & Moseley, 2013).

  • Evaluation

Physical assessment of the eyes: The physical examination of the eye should assess gross lesions, conformation, and symmetry (Sole, Klein & Moseley, 2013). The eye should be viewed in a place where there is good lighting and with minimal restraint to the head. The pupillary light reflexes and the anterior ocular segment are examined in detail with strong light and under magnification in a darkened room. Tests like fluorescein staining, Schirmer tear test, and intra ocular pressure measurement are also carried out.

Physical assessment of ears; In the ear examination data, information on discharge, earaches, infections, hearing loss, vertigo, environmental noise, tinnitus, and self-care should be obtained. An otoscope is used in caring out the ear examination. It gives a clear view of the eardrum and the ear canal (Sole, Klein & Moseley, 2013). When a pneumatic otoscope is used, it helps the doctor examine the movement of the eardrum.
