History Present Illness: The child developed a fever on 4th December 2012. She has a high temperature during night hours. She vomits frequently after sucking her mother's milk. She has skin rashes on her face and hands.
Significant Past History: The child has been suffering from fever frequently. She has been visiting the hospital and taking antibiotics. She suffered a fever for the first time in the fourth month. She developed skin rashes in the fifth month. After visiting the hospital and taking antibiotics the rashes and fever went away. 
Pregnancy: During pregnancy, the mother suffered from Malaria and headaches.  She also suffered from stomachache. After taking painkillers and antibiotics the complications went away.
Newborn Period: After birth, the mother suffered from stomachaches. She also had nausea and vomited
Illnesses: The mother suffered from malaria and minor complication like headache, nausea, and stomachache. The child suffered from fever in the fourth month and skin rushes in the fifth month.
Accidents: The mother and the child were never involved in the accident.
Preventive care: The mother takes pills which include antibiotics and pain killers. The child also takes pills.
Significant Family History: The mother has been developing hay fever for a long period. The father does not suffer from any genetic diseases. The parents are HIV negative.
Developmental History: The child developed the first complication during the fourth month. She developed fever and skin rushes in the fifth month. Since then she has been developing fever frequently.
Current Medical History: The child is using antibiotics while the mother is also using antibiotics and pain killers.
Allergies: The mother develops hay fever while the child has been suffering from fever severally.
Current Medications: Antibiotics which include aspirin

Significant Review of Systems: The child develops night sweat, fever, and rashes.
General: The child has lost weight and loses appetite.  
Skin: She has skin rashes while the mother does not have skin rashes.
HEENT: The head has soft hair, does not have ear disorders, the eyes are red and the nose and throat are clear.
Nose: The nose is clear.
Respiratory: It does not have a complication
Gastrointestinal: She does not have abdominal pain but has weight loss. She does not have problems with swallowing and digestion.
Urinary: Urinary includes symptoms like dysuria and nocturia.

Objective Data
Physical Exam: The child does not have complications in the body.
Vital Signs: The vital signs have no problem. He has a normal blood pressure of 95, respiratory rate, and heart rate.
Skin: Few skin rushes
HEENT: The head has soft hair, does not have ear disorders, the eyes are red and the nose and throat are clear.
Neck: Stiff neck
Lungs: Lungs do not have complications
Abdomen:  No complication in the abdomen
