Several Medical Record Formats at the Healthy Facility Record HIM (health Information Management) department is concerned with both internal and external facilities. It is too important for HIM to concentrate on both internal and external departments and not just on the information alone. This is because the needs of internal and external departments are interrelated and they should be met by HIM professionals who should be well versed with the new technology. The following are statistical data and report that is prepared by HIM department; a). Aggregate data which is prepared based on performance. b). patient centric data and is always related to the patients information. c). Transformed based data and is used for management and clinical support, planning and making of decisions. d).comparative data- is mainly used for health services research and outcome measurement. Numbering and filing systems used at the internship facility; Serial numbering Serial unit numbering system Unit numbering system Straight numeric filing Terminal digit filing Middle digit filing. Delinquency rate is applied to those employees who have failed to file all required registration documents, pay all interest, contributions or penalties and file all quarterly UC tax returns. The delinquency rate which is usually assigned to delinquent employers is regulated by the solvency measures which are only applicable in that particular year. The delinquency rate is the total sum of the basic rate + 3 percent + solvency measures. Whereby; basic rate is state adjustment factor +benefit ratio factor + reserve ratio factor. The solvency measures are additional contribution + surcharge adjustment + interest factor. The main reason for having a policy in any healthy facility is that this policy is intended to help the EMS agency in development of the retention policy. Records retention requirements are varied depending on the type of ownership of the EMS service. For instance all agencies which are owned by the local government re supposed keep a defined general retention and disposition schedule. The retention policy should give a guideline on how the records should be retained by all kinds of EMS services. All EMS agencies are supposed to have policy which shall describe all the procedures which are in compliance with the retention of records required. The policy should also describe the time length needed to document the records. Additionally, the policy should have described well how the document will be stored, procedures for retrieving the stored document and how the records will be protected. The following are the physical/ technical measures which are used to secure HIM department; both internal and external departments should be taught the importance of computer security, one should ensure that all other staffs take seriously security he or she does, ensure that all the information in the system components are catalogued because there are no two medical practices which have the same information, one should be ready for the disaster even before it has struck this means there should be an alternative method of backing up all the information in the system. Lastly, one should ensure that all his or her communications and network safeguards are robust and intact. The following are the both physical and technical measures which should used to secure health records; there should be facility access controls such as alarms and locks, there should be proper policies in the work station to make sure that there is proper access and use workstations, workstation security measures, such as computer privacy filters and cable locks, there should be a good plan on how to restore lost data. In order to ensure that records released to other entities are not reproduced there should be measures which are put into place to ensure that all the information in that record is fully protected. The following are the tracking methods which are mainly used by healthy facilities; a) Computerized tracking system, this require the facility to develop database and spreadsheets which will collect the required information for accounting purpose. b). manual log, this method is also known as paper log and one log is kept per person for ease in finding a file containing the report upon a request. c). authorization form, it ensures all the information required is included in the form. The most important office machine in the HIM department is the computer because it used to store and records all data regarding the healthy facility and the patients. There several areas in HIM those are outsourced. These areas are; contractors and consultants, data technicians, and also programmers. The reason for outsourcing these services is that it is a bit cheaper to outsource than to employ permanent employees in the facility. HIM professionals also turn on outsourcing so that they may obtain expert knowledge and also to enhance day to day operations of the facility. The following are areas which are within HIM department that needs to be outsourced; specialized and technical tasks such as transcription and coding services. Benefits of working in health facility are; there is an opportunity to help others, the industry is growing too fast thus healthcare industry is booming comparing with other industries, it is flexible in regard to the working hours, it has a fast paced and a challenging work environment thus making it interesting to work and lastly it has excellent benefits and pay. Hard skills are proficiencies that which are easily measured and can be acquired through learning, either during a career or schooling while on the other hand soft skills are skills which are not easy to measure. They range from personal traits to patience to organization. Hard skills and soft skills are of great importance to health facility because they help in running daily activities in the organization. It is of great importance for healthcare professional to know about current trends and issues in the health care facility because in the current world technology is changing on daily basis so they need to be equipped with the new technology and also so that they are up date with all the matters which patients are facing and be in position to handle cases with ease. The cost of health services is increasing and becoming more expensive because of the factors which are driving up hospital costs such as increase in the cost of services and goods which are used in healthcare. For example equipments, workers, and information system cost have greatly increased. The second factor is that some procedures which should be performed on a patient are of high cost thus increasing the cost of the service. And lastly is due to the increase in the cost of drugs. Four ways in which baby boomers population are affecting the health care industry: a). increase in the numbers of old people who are backing out of work force. b). increase in tension in work place because no new generation to who have skills to work on that area of expertise. c). it hard to retain experience which is of great importance in the health industry. d). the health care facilities are lacking new talents. Universal healthcare is one of the most controversial issues which are associated with health reforms because many people do not differentiate between the two they think they all mean the same. The second issue is many people understand that the healthcare system does not work effectively and efficiently though they do not know the scope of the problem involved. Continuous quality improvement (CQI) is a continuous and systematic action which is lead to improvement in the healthcare services and the health status of patients. Two quality improvement goals are; improved patients health and improved efficiency in clinical and managerial process. Sentinel event is an event which is mainly focusing on the patient’s safety though not necessary to what was the main cause of the patient’s illness or the underlying condition. The connection between sentinel event and the patient’s safety is that it analyses any condition which might reach the patient whether it is permanent harm, death or intervention needed to save a life. Trends in the supply and demand of health worker are there is growing pressure to get cost efficient and effective work force so they vary depending on the health care team and composition in question. Also there is change in the focus of healthcare delivery system. Advantages of EMR a). leads to faster decision making and care responses from the medical professional assigned. b). improved treatment and diagnosis. c). decrease in errors found within patients health records. d). quick transfer of patients data from one department to the other. e). improved results management. Disadvantages of EMR a). lack of privacy. b). Occurrence of identity theft. c). loss of data due to crashing of computers. I will buy the idea of having the data on my flash and working on it at my home place. It will give me option of working on the data even at late hours or overnight. So I will be able to beat the deadline. I think the new employee lacks soft skills because hard skills, he has them as he went to school and he is well equipped with the new tech knowledge but his personal traits are not worthy for the company as he is too mean. As patients health is of great concern to any health facility, before any other patient is treated in the machine, the machine should be thoroughly inspected for any failure and fixed. I think I should speak up because if the patient visits the hospital with the same complication I will be guilty for my silence. I think I will have to call for a workmate who will come and replace me so that there is no deficit in the number of staff in the facility because of my mistakes. Work cited Gates Mary.
