In the aetiology of complications of diabetic pregnancy, the disturbances of intermediary metabolism play a major role, in addition to abnormalities of immune function in both the mother and foetus, as well as in the placenta ( Fallucca F, 1997). The metabolic changes in insulin-dependent diabetes mellitus include postprandial hyperaminoacidemia, hyperlipidemia, and hyperglycaemia. (Reece EA, Homko C, Wiznitzer A, 1994.)
In particular, maternal hyperglycaemia results in foetal hyperglycaemia, which is corrected by increasing insulin secretion and consequent foetal hyperinsulinism. Embryo-foetal pathology occurs due to modified concentration of foetal nutrients and hyperglycaemia. The main features are malformation, hypertrophic cardiomyopathy, placental insufficiency, pre-eclampsia, macrosomia, foetal intrauterine death, delay in foetal lung maturity, hypoglycaemia, and neonatal hypocalcaemia. Pregnant women with diabetes should be seen at the earliest in a combined clinic comprising of a team that includes an obstetrician and a physician with a special interest in diabetes and pregnancy, a specialist diabetes dietician, a specialist diabetes nurse and a specialist midwife. It must be emphasized to the patient that pregnancies in women with diabetes are regarded as high-risk pregnancies and that they might need more frequent visits to the antenatal clinic. The frequency of visits will depend on an individual basis. (Care Recommendation, 2005).
