The continuity of health care is the consistence of the health care services provided to the needs of the patients. The rhyming of the needs of the health care providers and the health care recipients enhances prompt recuperation of the patients and the promotion and maintenance of quality health care.

According to Halloran and Lowenstein (2010), continuum of health care is a concept, which supports the sacred nature of human life. The understanding is vital because some patients are overwhelmed by their illnesses and opt for euthanasia.

The patients should be hospitalized for some time before the long term care in the long term health facilities. The conditions should also match.
