Participation in the study was dependent on the choice of the person, no force or repeated pestering for participation was made. 3. The diversity of the study group of both the physicians and patients was limited to those who chose to participate in the study. 4. The general ability of the obtained results from the collected data was limited to the sample of participants in this study.

Assumptions The following assumptions were made for this study: 1. The number of patients and physicians who participated in the present study were representative of all people of England. 2. The patients and physicians who stated of being belonging to England and indicating that they met all the requirements for the questionnaire, were actually correct. 3.
